Intermittent claudication: main symptom
The classic picture is recognizable: a person walks a certain distance, a squeezing, cramping pain appears in the calf, and he has to stop. After one or two minutes of standing, the pain goes away completely, and you can move on - exactly the same amount. The distance is reproducible: today, tomorrow and in a week the pain appears in approximately the same place, and uphill and with a fast step - earlier.
- Pain or cramping in the muscles when walking, disappearing with rest within 1–2 minutes
- Constant distance after which pain occurs and its reduction over time
- Coldness of the feet, feeling of coldness in the leg even when warm
- Hair loss on the lower legs, thinning and dry skin, thickened, slow-growing nails
- Pallor of the foot when raising the leg and purple coloring when lowering it
- In men with damage to the iliac arteries - erectile dysfunction
It is also worth knowing about the “silent” variant of the disease. People with diabetes and severe neuropathy may experience no pain at all when walking: damaged nerves simply do not transmit the signal. In sedentary older people, a person does not travel the distance at which pain would have time to appear. In both cases, the first manifestation is an immediate non-healing ulcer on the foot, so in these groups the arteries are checked routinely, without waiting for complaints.
Stages: from asymptomatic to critical

- Третья стадия — боль в покое, чаще ночью, когда исчезает помощь силы тяжести. Человек свешивает ногу с кровати или спит сидя, потому что так становится легче.
- Четвёртая стадия — трофические изменения: незаживающие язвы, некроз пальцев, гангрена. Это критическая ишемия, и её лечат срочно.
The third and fourth stages are united by the concept of critical ischemia - a condition in which there is not enough blood flow even at rest. Without restoration of blood flow, the risk of losing a limb within a year is very high, so such patients do not need a scheduled appointment, but an urgent consultation with a vascular surgeon.
Survey
Diagnosis here is inexpensive and quick: the main part of the information is provided by an examination with a pulsation check and a simple measurement of the pressure on the ankle with a regular cuff. Expensive studies are needed not to make a diagnosis, but to plan an intervention when it is already being discussed. Therefore, the excuse “it’s expensive to get examined” does not work in this case.

- Дуплексное ультразвуковое сканирование артерий: показывает локализацию, протяжённость сужений и скорость кровотока.
- КТ-ангиография или МР-ангиография — при планировании вмешательства, для точной карты поражения.
- Оценка общего риска: липидный профиль, глюкоза и гликированный гемоглобин, артериальное давление, состояние сонных артерий и сердца.
Treatment without surgery: what really works
- Полный отказ от курения. Это не общая рекомендация, а самый действенный шаг: курение ускоряет рост бляшек, вызывает спазм сосудов и резко ухудшает результаты любых операций на артериях. Продолжающий курить теряет проходимость шунта или стента в разы быстрее.
- Лечебная ходьба по протоколу: идти до появления умеренной боли, остановиться до её исчезновения, продолжить. Сеанс 30–45 минут, не менее 3–5 раз в неделю, курс не менее 12 недель. Именно нагрузка на грани боли заставляет развиваться обходные пути кровотока — щадящий режим такого эффекта не даёт.
- Статины — назначаются практически всем при этом диагнозе, независимо от исходного уровня холестерина: они стабилизируют бляшку и снижают риск инфаркта и инсульта.
- Антиагрегантная терапия по назначению врача для профилактики тромбоза.
- Жёсткий контроль артериального давления и сахара крови, снижение веса.
- Ежедневный уход за стопами и правильная обувь: при плохом кровотоке любая мелкая ранка заживает долго и легко превращается в язву.
It is worth mentioning separately what you should not expect from treatment. Vasodilator drips “for prevention” twice a year do not replace smoking cessation, statins, or walking, and have virtually no effect on the prognosis. It's the boring, everyday things that give real results.
When is surgery needed?
Intervention is discussed when lameness seriously limits life despite 3-6 months of conservative treatment, and necessarily and without delay - in case of critical ischemia: pain at rest, ulcers, necrosis. There is only one task - to restore blood flow below the blockage.
- Balloon angioplasty with stenting - through a puncture of the artery; low-traumatic, quick recovery, works well with short contractions
- Bypass - creating a bypass with your own vein or prosthesis; used for extended blockages and in cases where intravascular intervention is impossible
- Endarterectomy - removal of plaque from an artery, usually in the femoral area
- Hybrid operations combining both approaches in one intervention
It is important to understand what surgery does not do. It does not eliminate the cause and does not cancel treatment: after the intervention, the person continues to take statins and antiplatelet drugs, control blood pressure and sugar, and, most importantly, walk. The absence of pain when walking immediately after surgery creates a deceptive feeling of recovery - in fact, it is during this period that it is determined how long the restored blood flow will last. Long-term results in non-smoking patients who comply with therapy differ significantly from the results in those who continue to smoke.
Costly mistakes
A significant portion of severe cases arise not because of the particular aggressiveness of the disease, but because of habitual, seemingly harmless actions. Below is what most often brings patients to a vascular surgeon in a worse condition than could be with the same diagnosis.
- Warm the sore leg with a heating pad or a hot bath: with poor blood flow and reduced sensitivity, this is a direct path to burns and ulcers
- Consider pain when walking as an age-related norm and not get examined for years
- Treat “vessels” with ointments and rubs, postponing a visit to the angiologist
- Stopping statins due to liver fears without discussing it with your doctor: the risk of withdrawal is disproportionately higher
- Quit smoking “after surgery” - the correct order is reversed
- Wear tight shoes, trim calluses yourself, walk barefoot
- Stop walking due to pain: decreased activity worsens both blood flow and prognosis
