What defects are most common?
More than a hundred defects have been described, but a few variants account for the bulk of cases. They are usually divided into “pale” - when blood is discharged from the left parts of the heart to the right and there is no cyanosis, and “blue” - when venous blood enters the systemic circulation and the skin acquires a bluish tint.
- Ventricular septal defect is the most common defect; minor defects often close on their own in the first years of life
- Atrial septal defect is often asymptomatic and is detected already in preschool or school age
- Patent ductus arteriosus is a vessel that should close after birth, but remains open; especially typical for premature babies
- Coarctation of the aorta is a narrowing of the aorta; the key sign during examination is a weakened pulse in the legs with a good pulse in the arms
- Pulmonary valve stenosis and aortic valve stenosis
- Tetralogy of Fallot is a classic “blue” defect of four components
- Transposition of the great vessels is a critical defect that requires assistance in the first days of life
How to suspect a defect in a child
Signs depend on age and type of defect. In a newborn, the heart “works” primarily during feeding - which is why feeding becomes the main test of endurance.
- In newborns and infants: cyanosis, shortness of breath, sweating and pauses when sucking, poor weight gain, frequent bronchitis and pneumonia
- In children 1–3 years old: delay in physical development, rapid fatigue in outdoor games, desire to squat while running
- In preschoolers and schoolchildren: shortness of breath on exertion, chest pain on exertion, fainting, dizziness
- At any age: heart murmur first heard by a doctor
- Sometimes the defect does not manifest itself at all and becomes an incidental finding during echocardiography.
When the defect is found: before and after childbirth
- Скрининговое УЗИ плода на 18–22 неделе беременности. На этом сроке специалист прицельно осматривает сердце, и большинство серьёзных пороков видно уже здесь.
- Расширенная эхокардиография плода — при подозрении на сроке скрининга, при пороках у родственников, при диабете или перенесённой инфекции у матери.
- Пульсоксиметрия в роддоме — простое измерение насыщения крови кислородом на руке и ноге, помогает выявить критические «синие» пороки до появления симптомов.
- Осмотр педиатра и неонатолога: выслушивание шума, оценка пульса на бедренных артериях, цвета кожи, характера дыхания.
- Эхокардиография — метод, который ставит окончательный диагноз: показывает анатомию, размер дефекта, направление и объём сброса крови, давление в лёгочной артерии.
What examination is needed
- Эхокардиография с допплером — основное исследование; безболезненно, без облучения, при необходимости повторяется многократно.
- ЭКГ — оценка ритма, перегрузки отделов сердца, нарушений проводимости.
- Пульсоксиметрия и измерение давления на руках и ногах — разница указывает на коарктацию аорты.
- Рентгенография грудной клетки — размеры сердца и состояние лёгочного кровотока.
- КТ или МРТ сердца — при сложной анатомии и перед операцией.
- Катетеризация полостей сердца — при необходимости точного измерения давления и в качестве лечебной процедуры.
- Консультация генетика — при сочетании порока с другими особенностями развития.
Treatment: observation, catheter or surgery
The tactics are determined not by the name of the defect, but by its effect on the child’s blood circulation and growth. There are three possible scenarios, and all of them are full-fledged management options.
- Observation - for small defects without hemodynamic disturbances. The child is regularly examined by a cardiologist with a control echocardiogram, and some of these defects close on their own
- Endovascular closure - through a puncture of the vessel on the thigh, an occluder or coil is brought to the heart. This is how many atrial septal defects and patent ductus arteriosus are closed; the chest is not opened, recovery takes days
- Balloon procedure - expansion of a narrowed valve or section of a vessel with a catheter
- Open surgery - for complex and large defects, usually under artificial circulation
- Staged surgery - for complex defects, correction is carried out in several interventions as the child grows
- Drug support - before surgery or as part of treatment for heart failure, always as prescribed by a cardiologist
The timing of correction is determined in advance and discussed with the family. Postponing the operation “until he grows up,” if it is recommended at a particular age, is dangerous: with large discharges of blood, an irreversible increase in pressure in the vessels of the lungs develops, and then the defect becomes inoperable. That is why the appointed period of intervention is not a formality.
Life of a child with a defect and after surgery
- Прививки детям с пороками сердца делают по общему календарю — порок сам по себе не является противопоказанием, а инфекции для такого ребёнка опаснее, чем для здорового. Отвод даёт врач и только на конкретный период.
- Питание и режим обычные. Грудным детям с сердечной недостаточностью иногда рекомендуют более частые кормления меньшими объёмами — так ребёнок меньше устаёт.
- Физическая активность разрешена почти всегда, но её уровень определяет кардиолог. Полное освобождение от движения детям с корригированным пороком, как правило, не нужно и вредит.
- Профилактика инфекционного эндокардита: тщательный уход за зубами, своевременное лечение кариеса, информирование стоматолога о пороке.
- Наблюдение продолжается и после успешной операции — с индивидуальным интервалом ЭхоКГ и ЭКГ.
- Подростку с корригированным пороком объясняют его диагноз: во взрослом возрасте наблюдение продолжается уже у взрослого кардиолога.
