INR under control
Patients on warfarin take their INR regularly: leaving the target corridor risks thrombosis or bleeding, doses are adjusted according to the figure. The test also falls first in case of liver failure (coagulation factors are synthesized by the liver) and is included in the preoperative minimum. The result is on the day of delivery - the dose is adjusted without delay.
When is it appointed?
- preparation for operations is a mandatory block;
- taking anticoagulants (warfarin - INR) - dose control;
- thrombosis and miscarriage - search for thrombophilia;
- bleeding: bruises, nose, gums;
- pregnancy - planned control of hemostasis.
Coagulation system
Hemostasis balances between bleeding and thrombosis: screening tests (prothrombin/INR, aPTT, fibrinogen) evaluate the coagulation cascade, D-dimer catches active thrombus formation, special tests (lupus anticoagulant, genetics of thrombophilia) look for the cause of repeated thrombosis and pregnancy losses. Before any operation, a coagulogram is mandatory - hidden disorders are detected before the incision, and not during it.
