Preoperative standard
Three tests answer the question “is it possible to operate”: lengthening the indicators before surgery allows you to adjust medications (withdrawal/reversal of anticoagulants according to the protocol) or make up for deficiencies, eliminating bleeding on the table. In case of deviations without an obvious cause, the block is expanded to the hemostasiogram; interpretation - taking into account the medications taken.
When is it appointed?
- preparation for operations is a mandatory block;
- taking anticoagulants (warfarin - INR) - dose control;
- thrombosis and miscarriage - search for thrombophilia;
- bleeding: bruises, nose, gums;
- pregnancy - planned control of hemostasis.
Coagulation system
Hemostasis balances between bleeding and thrombosis: screening tests (prothrombin/INR, aPTT, fibrinogen) evaluate the coagulation cascade, D-dimer catches active thrombus formation, special tests (lupus anticoagulant, genetics of thrombophilia) look for the cause of repeated thrombosis and pregnancy losses. Before any operation, a coagulogram is mandatory - hidden disorders are detected before the incision, and not during it.
