Infective endocarditis
Infectious endocarditis is an inflammation of the inner lining of the heart and valves caused by bacteria or fungi that have entered the blood. Loose deposits of microbes, platelets and fibrin form on the valves; they destroy the valve and come off in pieces, clogging the vessels of the brain, kidneys, spleen and limbs. The disease is life-threatening and requires hospital treatment. It should be suspected in case of prolonged unexplained fever with chills, especially in people with a valve defect or prosthesis, after dental and surgical interventions, with intravenous drug use and the presence of an indwelling vascular catheter. Key studies are blood cultures and echocardiography.

Calcification of heart valves
Calcification of heart valves is the deposition of calcium salts in the leaflets and fibrous ring of the valve, due to which the tissue loses its elasticity. The most commonly affected areas are the aortic valve and the mitral valve annulus. At an early stage, the valves only become denser, the blood flow is not impaired, and the person does not feel anything - the finding is accidentally described on EchoCG. Over time, the valves can grow together and stop opening completely, and stenosis forms: the heart has to push blood through the narrowed opening. Then shortness of breath on exertion, chest pain and fainting appear. The process does not resolve with pills, but its progress can be slowed down and surgery can be offered in time.

Cardioneurosis (cardiac neurosis)
Cardioneurosis is a condition in which a person experiences pain in the heart area, irregularities and palpitations, but examination does not find heart disease. This is not fiction or pretense: the complaints are real, but are caused by a dysregulation of the autonomic nervous system against a background of anxiety and stress. Usually it all starts with fear for one’s heart, then the person listens to every beat, anxiety intensifies the symptoms, and the circle closes. The diagnosis is made only after the cardiologist has ruled out coronary artery disease and arrhythmias. Treatment focuses on anxiety and lifestyle, not the heart.

Cardiosclerosis (post-infarction)
Post-infarction cardiosclerosis is the replacement of a dead area of the heart muscle with a connective tissue scar after a myocardial infarction. The scar forms within a few weeks and remains for life. Unlike a healthy muscle, it does not contract and does not conduct electrical impulses, so the heart works with a smaller margin of safety. With a small scar, a person can feel good, but with a large scar, heart failure, rhythm disturbances, and left ventricular aneurysm develop. In addition, the underlying disease remains - atherosclerosis of the coronary arteries, and therefore the risk of recurrent infarction. Regular use of prescribed medications, control of blood pressure and cholesterol, quitting smoking and cardiac rehabilitation significantly improve the prognosis.

Coarctation of the aorta
Coarctation of the aorta is a congenital narrowing of the main vessel of the body, most often immediately after the separation of the arteries supplying the head and arms. Due to the obstruction, blood has difficulty getting into the lower half of the body: in the vessels of the arms and head the pressure is high, and in the legs it is low. The left ventricle works under overload, its wall thickens. In newborns with severe narrowing, the condition worsens sharply in the first days of life when the ductus arteriosus closes. In older children and adults, the defect is often detected during examination for persistent arterial hypertension. Treatment is to eliminate the narrowing.

Pulmonary hypertension
Pulmonary hypertension is a persistent increase in pressure in the pulmonary artery, due to which the right ventricle has to work against increased resistance. It thickens, then expands and gradually ceases to cope, right ventricular failure develops with edema and enlargement of the liver. The main complaint is shortness of breath, which increases over months and is long explained by fatigue, excess weight or age. The causes are varied: diseases of the left side of the heart, chronic lung diseases, previous thromboembolism, systemic connective tissue diseases, a rare primary form. Correct diagnosis requires consistent testing because treatment depends entirely on the cause.

Myxoma of the heart
Myxoma is the most common benign tumor of the heart. Most often, it grows on a stalk from the interatrial septum in the left atrium and is a gelatinous formation that moves freely with the blood flow. The danger is not related to malignancy, but to mechanics: the tumor periodically blocks the mitral orifice, causing sudden shortness of breath and fainting, and its loose surface can come off in fragments and clog brain vessels. Myxoma often manifests itself with general symptoms - low-grade fever, weight loss, joint pain, which is why it has long been mistaken for another disease. Treatment is only surgical.

Myocarditis
Myocarditis is an inflammation of the heart muscle, most often caused by a common viral infection. This is its main feature: the disease begins not as a heart disease, but as a cold, and a person associates weakness and shortness of breath with a recent virus, and not with the heart. Most cases are mild and end with complete recovery; no one will ever know about them. But there is another side: it is myocarditis that remains one of the leading causes of sudden cardiac death in young people and athletes, and almost always due to physical activity to which the person returned too early. Therefore, this disease has one rule that is worth knowing before any diagnosis: after any infection with a fever, you cannot immediately return to heavy training, and if after the virus you develop chest pain, shortness of breath or a feeling of interruptions in the heart, this is a reason to be examined and not wait it out.

Mitral regurgitation
Mitral regurgitation is an incomplete closure of the mitral valve leaflets, due to which during ventricular contraction some of the blood returns back to the left atrium. This backflow is called regurgitation and is graded by echocardiography. Minor, physiological regurgitation occurs in healthy people and does not require treatment. Significant insufficiency forces the heart to pump excess blood volume: the atrium and ventricle gradually expand, pressure in the pulmonary vessels increases, shortness of breath, fatigue and rhythm disturbances appear. The causes include valve prolapse, previous rheumatism, coronary artery disease, infective endocarditis and heart enlargement due to cardiomyopathy.

Mitral stenosis
Mitral stenosis is a narrowing of the opening between the left atrium and the left ventricle, making it difficult for blood to flow into the ventricle. The pressure in the atrium increases, it expands, blood stagnates in the vessels of the lungs, and shortness of breath appears. The vast majority of cases are associated with acute rheumatic fever suffered in childhood or adolescence after streptococcal sore throat: the valve gradually scars, and the defect appears years later. A person feels normal for a long time, but the first signal is shortness of breath on exertion or an attack of atrial fibrillation. The diagnosis is made by echocardiography, treatment depends on the degree of narrowing.
