Secondary (symptomatic) arterial hypertension
Secondary arterial hypertension is an increase in blood pressure that has a specific, removable cause: kidney disease, narrowing of the renal artery, tumor or hyperfunction of the adrenal glands, disruption of the thyroid gland, sleep apnea, and taking certain medications. Unlike hypertension, in which the cause cannot be found, eliminating the source often normalizes blood pressure or sharply reduces the need for pills. This form is suspected when hypertension begins at a young age or after 60 years of age, is severe, difficult to treat, or is accompanied by unusual symptoms. The examination is prescribed by a doctor; taking hormones on your own is pointless.

High lower (diastolic) pressure
Lower, or diastolic, pressure is the pressure in the arteries when the heart relaxes between contractions. It largely reflects the tone of small vessels: the more they are narrowed, the higher the lower number. Diastolic pressure of 90 mmHg is considered elevated. Art. and higher when correctly measured at rest and confirmed again on different days. An isolated increase in lower pressure with normal upper pressure is more common in people under 50 years of age and is usually associated with excess weight, excess salt, lack of sleep, stress and alcohol. Less commonly, it is associated with diseases of the kidneys, adrenal glands or thyroid gland. Even without complaints, such pressure increases the risk of stroke and heart attack.

Hypertensive crisis
Hypertensive crisis is a sudden significant increase in blood pressure, usually up to 180/110 mmHg. Art. and higher, which is accompanied by a deterioration in health or threatens organ damage. A distinction is made between a complicated crisis, in which the brain, heart, kidneys or blood vessels are affected, and an uncomplicated crisis - without signs of such damage. A complicated crisis can manifest itself as a stroke, heart attack, pulmonary edema, aortic dissection and requires immediate call 103 and treatment in a hospital. Most often, crises occur in people with poorly controlled hypertension, when they skip or stop taking medications on their own, when they are stressed, or when they consume alcohol and large amounts of salt. The main prevention is regularly taking prescribed therapy and monitoring blood pressure at home.

Left ventricular hypertrophy
Left ventricular hypertrophy (LVH) is a thickening of the walls of the main pumping chamber of the heart, which develops as a response to prolonged increased load. Most often it is caused by arterial hypertension: the heart has to push blood against high pressure for years, and the muscle builds up mass. Other causes include narrowing of the aortic valve, hypertrophic cardiomyopathy, storage diseases, and in athletes, adaptation to training. At first, hypertrophy helps the heart cope, but over time, the thickened muscle becomes stiff, relaxes less well, and is less well supplied with blood. This increases the risk of heart failure, arrhythmias, heart attack and stroke. The good news is that with stable blood pressure control, hypertrophy can decrease.

Hypokalemia (low potassium)
Hypokalemia is a decrease in potassium levels in the blood. Potassium is responsible for the electrical activity of cells, so the muscles and heart suffer first of all: weakness, fatigue, cramps in the calves, constipation, and with a pronounced decrease - interruptions in the functioning of the heart, dangerous rhythm disturbances. Most often, the reason is not in diet: potassium is lost in the urine when taking diuretics, with vomiting and diarrhea, in diseases of the kidneys and adrenal glands. It is especially important to remember the connection with magnesium: until magnesium is replenished, potassium cannot be raised. Low potassium is especially dangerous for heart disease, and therefore requires the attention of a doctor.

Bicuspid aortic valve
A bicuspid aortic valve is a congenital condition in which the valve between the left ventricle and the aorta consists of two leaflets rather than three. It is one of the most common congenital heart defects, and many people live with it for decades without knowing it. However, such a valve experiences increased stress, wears out faster and calcifies earlier, so over time, a narrowing of the hole or reverse blood flow may develop. In some patients, the wall of the ascending aorta is simultaneously damaged - it expands and forms an aneurysm. That is why the defect does not require treatment, but regular monitoring by echocardiography.

Ventricular septal defect (VSD)
A ventricular septal defect is a congenital heart defect in which there is a hole in the wall between the right and left ventricles. Through it, blood under high pressure is discharged from the left ventricle into the right and again goes to the lungs, overloading the pulmonary circulation. The larger the defect, the greater the burden: the child sucks worse, sweats during feeding, does not gain weight well, and more often suffers from bronchitis. Minor defects often close on their own in the first years of life and do not interfere in any way. The diagnosis is made by echocardiography, and the decision about surgery is made by a pediatric cardiologist together with a cardiac surgeon.

Left ventricular diastolic dysfunction
Left ventricular diastolic dysfunction means the heart is less able to relax and fill with blood between beats, although it pumps it out normally. The muscle becomes more rigid and higher pressure is required to fill the ventricle; it is transmitted to the left atrium and pulmonary vessels, causing shortness of breath on exertion. Most often, such a conclusion appears with long-term hypertension, in older people, with obesity and diabetes mellitus. A mild degree in an elderly person without complaints is largely an age-related feature. But with shortness of breath and edema, we are talking about heart failure with preserved ejection fraction, which needs to be treated.

Additional (false) chord of the left ventricle
The accessory, or false, chord of the left ventricle is a thin connective tissue cord stretched inside the cavity of the ventricle between its walls. Unlike real chordae, it is not associated with the mitral valve leaflets and does not participate in its operation. This is a structural feature of the heart that is formed before birth and refers to minor developmental anomalies. It is found accidentally during echocardiography, often after the doctor hears a low noise in the child. Such a chord has virtually no effect on the pumping function of the heart: the chambers do not expand, the valves work normally, and blood circulation does not suffer. In most cases, treatment is not required, and restrictions in physical education and sports, as a rule, are not necessary.

Fluid in the pericardium (pericardial effusion)
The pericardium is a dense, two-layered sac around the heart. Normally, there is a little fluid between its leaves, which reduces friction during contractions. If the fluid increases, it is referred to as pericardial effusion. A small amount is often found by chance on echocardiography, and it may not manifest itself in any way. The danger is created not so much by the volume as by the rate of accumulation: with the rapid intake of even a moderate amount of fluid, the pericardium does not have time to stretch, compresses the heart and prevents it from filling with blood. This is tamponade, a life-threatening condition. Therefore, effusion always requires identification of the cause and evaluation by a cardiologist.
