Rheumatism (acute rheumatic fever)
Acute rheumatic fever, which is commonly called rheumatism, is an inflammatory disease that develops 2-4 weeks after a sore throat or pharyngitis caused by group A beta-hemolytic streptococcus. The immune system, attacking streptococcus, mistakenly damages its own tissues with a similar structure: the heart, joints, nervous system, skin. Children and adolescents aged 5 to 15 years are most often affected. The main danger is heart damage (rheumatic carditis), which can lead to the formation of chronic rheumatic heart disease with valve defects. Joint pain goes away without a trace, and valve changes remain for life. Timely treatment of streptococcal sore throat with antibiotics and prevention of repeated attacks can prevent these consequences.

WPW (Wolf-Parkinson-White) syndrome
Wolff-Parkinson-White syndrome is a congenital feature of the conduction system of the heart, in which an extra muscle bridge, an additional conduction path, remains between the atria and ventricles. The impulse travels along it, bypassing the atrioventricular node, so part of the ventricle is excited ahead of schedule, and characteristic changes appear on the ECG. This path in itself does not interfere with life, but it can close the impulse in a circle and trigger an attack of very rapid heartbeat. If changes are visible only on film and there are no attacks, they speak of the WPW phenomenon; if there are attacks, talk about the syndrome. The condition is completely treatable with catheter ablation.

Brugada syndrome
Brugada syndrome is a hereditary disorder of the ion channels of the heart muscle. Structurally, the heart appears healthy, but the electrical impulse in the right ventricle is not conducted correctly, and the characteristic ST segment elevation in leads V1–V2 appears on the ECG. The main danger of the syndrome is attacks of ventricular tachycardia and ventricular fibrillation, which manifest themselves as sudden fainting, and in the worst case, cardiac arrest. Attacks most often occur at night, during sleep, at rest, or against a background of high fever. Mostly young and middle-aged men are affected. The syndrome is inherited, so after identification, all close relatives are examined.

Marfan syndrome
Marfan syndrome is an inherited connective tissue disorder caused by a change in the fibrillin-1 gene. This protein gives strength and elasticity to the walls of blood vessels, ligaments, the lens of the eye and the membranes of the joints, so several systems are affected at once. In appearance, people with the syndrome are usually tall and thin, with long limbs and fingers, flexible joints and chest deformities. The main danger is hidden inside: the aortic wall gradually stretches, and if left unattended, it can rupture or dissect. That is why the diagnosis is important not in itself, but as a basis for regular heart monitoring and timely surgery, which today allows you to live a long time.

Early ventricular repolarization syndrome
Early ventricular repolarization is an electrocardiographic finding in which the ECG region after the ventricular complex is elevated above normal levels. This picture occurs in many healthy people, especially young men, athletes and people with a low pulse, and most often reflects a feature of the restoration of the electrical charge of heart cells, and not a disease. It usually does not cause complaints and is discovered by chance during a medical examination or before surgery. Rare cases are considered separately when a similar picture is combined with fainting or a family history of sudden death: then a more careful examination by a cardiologist is needed.

Sick sinus syndrome
The sinus node is the natural pacemaker of the heart, which sets the frequency of contractions and accelerates it during exercise. With sick sinus syndrome, it stops coping: the pulse becomes too rare, pauses occur between contractions, and the heart does not accelerate when walking and physical work. Because of this, the brain receives less blood, weakness, darkening of the eyes, dizziness and fainting appear. Sometimes a rare rhythm alternates with attacks of rapid heartbeat. Most often, the condition is associated with age-related changes in the conduction system. The diagnosis is confirmed by daily ECG monitoring, and if symptoms are severe, a pacemaker is installed.

Long QT syndrome
Long QT syndrome is a condition in which the heart muscle takes too long to recover between contractions. This is visible on the electrocardiogram as prolongation of the QT interval. The danger is that at such a vulnerable moment a special type of ventricular tachycardia can start, causing fainting, and in severe cases, cardiac arrest. The syndrome can be congenital, associated with changes in ion channel genes, or acquired, most often due to taking drugs that prolong QT, or potassium and magnesium deficiency. Typical situations when fainting occurs are physical activity, sudden fear, loud sound, and in some patients, sleep. The condition is well controlled with proper management.

Sinus arrhythmia
Sinus arrhythmia is an irregular heart rhythm in which the impulses still originate from the sinus node, but the intervals between beats are slightly different. Most often this is a respiratory arrhythmia: when you inhale, the pulse quickens, when you exhale it slows down. This fluctuation reflects the normal influence of the autonomic nervous system on the heart and is especially pronounced in children, adolescents and young adults. Therefore, the entry “sinus arrhythmia” in the ECG conclusion of a child without complaints usually does not mean illness. Less common is non-respiratory sinus arrhythmia, which occurs in the elderly, with heart disease or taking certain medications. An examination is needed if the irregular rhythm is combined with fainting, shortness of breath, chest pain or severe weakness.

Pulmonary stenosis
Pulmonary stenosis is a congenital narrowing of the pathway through which blood from the right ventricle enters the lungs. Most often, the valve itself is narrowed: its flaps are fused and do not open completely. To push blood through a narrow opening, the right ventricle works with increased pressure, its wall thickens, and with prolonged overload, the pumping function begins to suffer. Mild forms are common, do not manifest themselves in any way and are detected by a heart murmur. Severe narrowing in newborns can lead to cyanosis and requires treatment in the first days of life. The main diagnostic method is echocardiography.

Tetralogy of Fallot
Tetralogy of Fallot is the most common congenital heart defect accompanied by cyanosis. It has four components: narrowing of the right ventricular outlet, a large ventricular septal defect, displacement of the aorta that lies above the defect, and thickening of the right ventricular wall. Due to the obstruction on the way to the lungs, part of the venous blood enters through the defect directly into the aorta and spreads throughout the body, bypassing the lungs - the skin acquires a bluish tint. The severity depends on the degree of narrowing. The defect is recognized in the first months of life, and sometimes even before birth, and treated surgically.
