Dynamic cavernosography
A vasoactive drug is injected into the corpus cavernosum (as in a pharmacotest), after the development of an erection, contrast is slowly applied through the same needle, and photographs are taken in frontal and lateral projections; in case of venous leakage, the contrast quickly fills the deep dorsal, pedunculate or circumflex veins - this is what determines the extent of the operation. At the same time, intracavernosal pressure can be measured (cavernosometry). After the study, the erection is removed if necessary by administering phenylephrine, the patient is observed until it subsides and goes home.
When is it prescribed?
Cavernosography is an X-ray contrast examination of the corpora cavernosa in cases of suspected venous leakage as a cause of erectile dysfunction (before surgery on the veins), in Peyronie's disease with deformation, cavernous fibrosis after priapism, or trauma to the penis. Genitography - contrasting the internal genital tract for developmental anomalies (urogenital sinus, intersex conditions, hypospadias with utriculus), fistulas and cysts for planning reconstructive surgery.
Security
Both studies are performed with iodinated contrast under local anesthesia and X-ray control with a minimal dose of radiation. Contraindications: allergy to iodine, acute inflammation. After cavernosography, the erection is relieved by administering the drug if necessary.
