What is the difference between an initial appointment and a repeat appointment?

The initial appointment is the most complete: the doctor meets the patient for the first time, so he spends time on a detailed collection of complaints and history, a full examination and diagnostic planning. It is at the initial appointment that the decision is made whether ultrasound, tests, cystoscopy or MSCT are needed. A repeat appointment is shorter and cheaper - it is needed to analyze the results and correct the treatment; if you contact the same doctor with a new problem a year or more later, the appointment is again considered a primary one.

When to make an appointment with a urologist

  • frequent, painful or difficult urination, pain, feeling of incomplete emptying;
  • blood in the urine, cloudy urine, unusual odor;
  • pain in the lower back, lower abdomen, perineum or scrotum;
  • discharge from the urethra, itching, rashes on the genitals;
  • decreased potency, premature ejaculation, pain during sexual intercourse;
  • urinary incontinence, night urges, weak stream in men after 45 years;
  • stones in the kidneys or bladder according to ultrasound;
  • infertility in a couple, changes in spermogram.

For men over 45 years of age, a preventive examination by a urologist is recommended once a year, even without complaints: PSA, ultrasound of the prostate and uroflowmetry allow timely detection of adenoma and prostate cancer.

What is included in the initial appointment

The doctor asks in detail about the complaints, their duration, past illnesses, operations and medications taken. Then he conducts an examination: palpates the area of the kidneys and abdomen, in men examines the external genitalia and, if indicated, performs a digital rectal examination of the prostate - this takes less than a minute and allows you to assess the size and density of the gland.

If the clinic has an ultrasound room, the doctor immediately performs an ultrasound examination of the kidneys, bladder with determination of residual urine, prostate (transabdominal or TRUS) or scrotal organs. If there are complaints about a weak stream, uroflowmetry is performed. Based on the results of the appointment, the patient receives a preliminary diagnosis, a list of tests and the first prescriptions.

What examinations does a urologist prescribe?

Basic set: general urine test, urine culture for flora, general and biochemical blood test, PSA in men after 45 years. According to indications - tests for sexually transmitted infections (PCR), spermogram, hormonal profile, MSCT of the kidneys without contrast for stones, cystoscopy, urethroscopy, excretory urography. Most tests are performed in the clinic on the day of your visit.

How to prepare for your appointment

Take the results of past tests, ultrasound, discharge notes and a list of medications. For 1-2 days, abstain from sexual intercourse and alcohol. If you are planning a bladder ultrasound, drink 0.5–1 liters of water an hour before and do not urinate. You can take a morning general urine test in advance. Women are not examined by a urologist during menstruation unless it is an emergency.

Repeated appointment

At a follow-up appointment, the urologist evaluates the examination results, makes a final diagnosis and prescribes or adjusts treatment. A follow-up consultation costs less than the initial consultation and is usually needed after 7-14 days. For chronic diseases (prostatitis, urolithiasis, adenoma), the doctor determines an observation schedule - usually once every 3-6 months.
