An increase 2–6 hours after the onset of pain with dynamics in paired tests confirms a heart attack even with an ambiguous ECG; A stable negative troponin at low risk allows one to safely rule out a catastrophe. Other conditions also give slight increases (myocarditis, pulmonary embolism, CKD) - cardiological interpretation. Chest pain = ECG + troponin immediately, do not “endure the night.”
When is it appointed?
- chest pain - rule out myocardial infarction;
- dynamics in acute coronary syndrome;
- control of myocardial damage after procedures;
- differentiation of muscle and cardiac causes of elevated enzymes.
The clock matters
For chest pain, troponin answers the main question - is there any myocardial damage: modern tests detect a heart attack 2-3 hours from the onset, paired measurements (0 and 1-3 hours) confirm or exclude the diagnosis. CPK-MB complements the picture in case of reinfarction and muscle “noise”. Negative markers with persistent pain do not cancel the ECG and the doctor - the vascular scenario is assessed comprehensively and urgently.
