Troponin lasts for days - if a REPEATED heart attack is suspected in the first days, it is CPK-MB with its rapid clearance that shows a fresh wave. The fraction also separates cardiac from musculoskeletal (training, injury, statins). Works in conjunction with troponin, not instead of it.
When is it appointed?
- chest pain - rule out myocardial infarction;
- dynamics in acute coronary syndrome;
- control of myocardial damage after procedures;
- differentiation of muscle and cardiac causes of elevated enzymes.
The clock matters
For chest pain, troponin answers the main question - is there any myocardial damage: modern tests detect a heart attack 2-3 hours from the onset, paired measurements (0 and 1-3 hours) confirm or exclude the diagnosis. CPK-MB complements the picture in case of reinfarction and muscle “noise”. Negative markers with persistent pain do not cancel the ECG and the doctor - the vascular scenario is assessed comprehensively and urgently.
