Aseptic necrosis of the femoral head
Aseptic necrosis of the femoral head is the death of a section of bone tissue due to disruption of its blood supply. The word "aseptic" means that there is no infection. The femoral head receives blood through a small number of vessels, and if the blood flow stops, the bone beams are destroyed, the head loses strength and gradually sags, and after it the joint collapses. The disease often begins at working age, and in the first weeks and months the X-ray remains normal, and the person is treated for a “pinched nerve.” This is why, for persistent pain in the groin and at risk groups, an MRI is needed: at an early stage, the joint can still be preserved.

Achondroplasia
Achondroplasia is the most common form of disproportionate short stature. The reason is a change in the FGFR3 gene, due to which the growth of cartilage in the growth areas of long bones is excessively inhibited. Bones that grow from cartilage - the shoulder, hip, base of the skull - are shortened, and those that are formed differently, such as the calvarium and vertebrae, suffer less. Therefore, they are characterized by short arms and legs with an almost normal body length, a large head with a protruding forehead and a depressed bridge of the nose. Intellect is completely preserved. The main task of observation is to notice complications in the respiratory, middle ear and spine in a timely manner and help the child live an active life.

Osgood-Schlatter disease
Osgood-Schlatter disease is the most common cause of knee pain in active teenagers. During the period of rapid growth, bones lengthen faster than muscles, and the powerful quadriceps femoris muscle, through its own patellar ligament, forcefully pulls the not yet strengthened growth zone at its attachment to the tibia. The repetitive pull of jumping, running and squatting causes irritation and micro-damage, causing tenderness and the characteristic hard bump to appear under the kneecap. The condition is benign and self-limiting: it resolves as the growth plates close, although the raised bump often remains permanently.

Perthes disease (osteochondropathy of the femoral head)
Perthes disease is a disruption of the blood supply to the head of the femur in a growing child, due to which part of the bone tissue temporarily dies and then gradually recovers. Boys aged 4–10 years are most often affected; usually one joint is affected. The process lasts for several years and goes through the stages of necrosis, fragmentation, recovery and outcome. The main danger is that the softened head can become deformed under load, and the abnormal shape of the joint will remain for life, leading to early coxarthrosis. The insidiousness of the disease is that the first sign is often not pain in the hip, but lameness and complaints about the knee. The earlier the diagnosis is made and the load regimen is selected, the higher the chance of maintaining the rounded shape of the head.

Ankle pain
The ankle joint bears the weight of the entire body with every step, so pain there is very common. The most common cause is the foot turning inward with a sprain or tear of the external ligaments: this is a classic sports and everyday injury. But pain can also occur without a fall: with inflammation of the Achilles tendon, arthrosis after long-term injuries, gout, flat feet, and in overweight people - due to chronic overload. A combination of circumstances and examination helps to figure it out: it is important whether there was an injury, whether it is possible to step on the foot, where exactly it is painful when pressed. These details determine whether an x-ray is needed and how to treat.

Wrist and hand pain
Pain in the wrist and hand is a very common complaint, with a variety of different causes behind it. The hand is complex: eight small carpal bones, many joints, tendons in narrow channels and nerves passing through tight spaces. Therefore, pain can come from a joint, tendon, nerve or ligament, and treatment is different in each case. The most common overload conditions occur in people who work a lot with their hands and at the keyboard: tendonitis, carpal tunnel syndrome, trigger finger. Injuries are considered separately, especially a fall on an outstretched arm, in which it is easy to miss a fracture of the scaphoid, and systemic joint diseases.

Knee pain
Knee pain is one of the most common complaints when visiting an orthopedist. The knee joint bears the weight of the body, works like a hinge and is supported by ligaments, menisci and muscles, therefore it is vulnerable to injury, overload, and age-related changes in the cartilage. In young people, pain is more often associated with sports, damage to the menisci and ligaments, or overload of the patella; After forty, osteoarthritis comes to the fore. A separate group of causes are inflammatory and metabolic diseases: gout, rheumatoid and reactive arthritis. Understanding the cause is important because the approaches to treatment for these conditions are fundamentally different.

Elbow pain
The elbow is a complex joint in which three bones are connected, and nearby there are tendons of the forearm muscles, ligaments, bursae and nerves. Therefore, pain in it is often associated not with the joint itself, but with the surrounding tissues. The most common cause in adults is inflammation of the tendons where they attach to bony protrusions: external epicondylitis, known as tennis elbow, and internal epicondylitis, known as golfer's elbow. Less common are bursitis, pinched ulnar nerve, arthritis and the consequences of injuries. It helps to understand the cause where exactly it hurts and what movements intensify it, and an examination and ultrasound determine the point.

Shoulder pain
The shoulder joint is the most mobile joint in the body, and for this mobility it pays with vulnerability. It is held not so much by bones as by the muscles and tendons of the rotator cuff, which are easily overloaded by work with raised arms, sports or monotonous movements. Therefore, most cases of shoulder pain are not related to the joint itself, but to the surrounding soft tissues. But pain in the shoulder can also be referred: it is caused by the cervical spine, gall bladder, lungs and heart. It helps to understand the source of how exactly the pain changes when you move your arm and neck.

Back pain
Most adults experience back pain at least once in their lives. In the vast majority of cases, it is nonspecific: the source is the muscles, ligaments and small joints of the spine, and not a hernia or “displacement of the vertebrae.” Such pain usually decreases significantly within a few weeks, and the main thing in its treatment is not bed rest, but maintaining feasible activity. It is much more important to recognize rare dangerous causes in time: fracture, infection, tumor, compression of nerve roots. They are the ones the doctor's examination is aimed at, and they are the ones who determine whether images are needed.
