Primary biliary cholangitis
Primary biliary cholangitis is a chronic autoimmune disease in which the immune system gradually destroys the small bile ducts inside the liver. Bile stagnates, damages liver cells, and over time, fibrosis develops, and without treatment, cirrhosis. The former name “primary biliary cirrhosis” was changed precisely because most patients today are diagnosed long before cirrhosis. Mostly middle-aged women are affected. The first manifestations are nonspecific: severe fatigue and skin itching, which is not explained by a skin disease. Often the disease is discovered accidentally by elevated alkaline phosphatase in a biochemical blood test.

Bend (deformation) of the gallbladder
A bend, or deformation, of the gallbladder is a change in its shape, which is detected by ultrasound: the bladder is curved in the neck or body, has a constriction or an S-shape. This conclusion frightens many parents and adults, but in most cases, the bend is a congenital anatomical feature or a temporary change in shape when the bladder overflows and a change in body position. It is not a disease in itself, does not require treatment and does not interfere with the flow of bile. Only those cases where there are typical complaints or the deformity is combined with stones, inflammation or impaired contractile function deserve attention.

Hepatic encephalopathy
Hepatic encephalopathy is a brain disorder that develops when a diseased liver stops neutralizing toxic metabolic products, primarily ammonia. In cirrhosis, blood from the intestines partially bypasses the liver through dilated veins and carries these substances directly to the brain. Sleep, attention, behavior and handwriting change, characteristic large tremors of the hands appear, and in severe cases, confusion and coma. Almost every episode is triggered by a trigger: bleeding from the esophageal veins, infection, constipation, dehydration, excess diuretics or alcohol. It is important to understand the main thing: the condition is reversible, with timely help consciousness is restored, so the first changes in behavior cannot be attributed to fatigue or age.

Barrett's esophagus
Barrett's esophagus is a condition in which the normal squamous epithelium of the lower esophagus is replaced by intestinal-type epithelium. This is a protective restructuring in response to many years of acidic stomach contents: the new mucosa is more resistant to acid, but its cells are more prone to further changes. The condition itself does not hurt and does not have specific symptoms, but it is detected during gastroscopy in people with prolonged heartburn. The significance of Barrett's esophagus is that it is considered precancerous: the risk of adenocarcinoma of the esophagus is increased, although the vast majority of patients never develop a tumor. Therefore, reflux treatment and regular monitoring are needed.

Child has poor appetite
Poor appetite in a child is one of the most common causes of anxiety for parents, and in the vast majority of cases there is no illness behind it. After a year, growth rates slow down sharply, and the need for food naturally decreases: a one-year-old baby needs much fewer calories per kilogram of weight than an infant. Children eat unevenly - today a lot, tomorrow almost nothing, and it is necessary to evaluate not one plate, but a week. The main guideline is not the amount of food eaten, but weight and height gain, activity and well-being. Real problems reveal themselves as weight loss, lethargy, pallor or other symptoms, and then you need a pediatrician.

Elevated liver enzymes (ALT, AST)
ALT (alanine aminotransferase) and AST (aspartate aminotransferase) are enzymes that are found inside liver cells, and AST is also found in muscles and the heart. When cells are damaged, enzymes are released into the blood and their levels in the test increase. Elevated transaminases are one of the most common reasons for visiting a gastroenterologist. The most common cause is fatty liver disease associated with excess weight, as well as alcohol, viral hepatitis B and C, medications and dietary supplements. Sometimes AST rises after intense exercise, unrelated to the liver. The degree of increase, the ratio of enzymes and other indicators help the doctor find the cause.

Increased bilirubin
Bilirubin is a yellow-brown pigment that is formed during the natural breakdown of red blood cells. The liver converts it into a form that is excreted with bile into the intestines. If too much bilirubin is produced, the liver does not have time to process it or bile cannot flow normally, its level in the blood rises, and with a significant increase, jaundice appears - yellowing of the skin and whites of the eyes. Elevated bilirubin is not a diagnosis, but a signal: the cause may be harmless Gilbert's syndrome, but also hepatitis, a stone in the bile duct, hemolytic anemia or a tumor. The ratio of direct and indirect fractions and other indicators help to correctly understand the analysis.

Stomach polyps
Gastric polyps are growths of the mucous membrane, which are most often discovered accidentally during gastroscopy performed for another reason. Most of them are small, do not cause complaints and are not dangerous, but different types of polyps can behave differently. Hyperplastic polyps are associated with chronic inflammation and H. pylori infection, fundic gland polyps often appear due to long-term use of acid-reducing drugs, and adenomatous polyps are considered precancerous and require removal. It is impossible to determine the type by appearance: the answer is given only by histological examination after biopsy or removal.

Polyps and cholesterosis of the gallbladder
Gallbladder polyps are any growths on its inner wall that are visible on ultrasound and do not move when changing body position. Most of them are not true tumors, but accumulations of cholesterol in the wall of the bladder (cholesterosis), which do not develop into cancer. Less common are true polyps - adenomas, which can become malignant over time. It is not always possible to distinguish them by ultrasound, so the tactics are determined by size, shape, growth rate and risk factors: small polyps are observed using control ultrasound, and large and growing ones are removed along with the gallbladder laparoscopically.

Postcholecystectomy syndrome
Postcholecystectomy syndrome is a collective name for complaints that persist or appear after removal of the gallbladder: pain in the right hypochondrium, bitterness in the mouth, nausea, bloating, loose stools. This in itself is not a diagnosis, but a signal that the cause of the discomfort needs to be looked for further. In approximately some patients, it turns out that the complaints were initially associated not with stones, but with another disease - gastritis, ulcers, reflux or irritable bowel syndrome. In others, they find a remaining stone in the duct, a narrowing, a disruption of the sphincter, or a change in the exchange of bile acids. The good news is that in most cases the cause can be found and corrected.
