Adrenal insufficiency (Addison's disease)
Adrenal insufficiency is a condition in which the adrenal glands do not produce enough cortisol, and often aldosterone. Cortisol helps the body cope with stress, maintains blood pressure and sugar levels, and aldosterone retains sodium and water. With their deficiency, weakness, weight loss, decreased blood pressure, craving for salty foods increase, and in the primary form, the skin darkens, especially in folds and scars. Complaints are non-specific and develop over months, so the diagnosis is often made late - sometimes only after a sharp deterioration. The main danger is adrenal crisis, a life-threatening condition that requires immediate help. With properly selected therapy, a person lives a normal life.

Diabetes insipidus
Diabetes insipidus is a rare disease in which the kidneys lose their ability to retain water, and the body excretes large volumes of light-colored urine. It has nothing to do with diabetes: the glucose level is normal, and the problem is with the hormone vasopressin. Either the pituitary gland stops producing it after injury, surgery, tumor or inflammation, or the kidneys stop responding to it. A person excretes from three to twenty liters of urine per day and is forced to drink the same amount, otherwise dehydration quickly sets in as sodium levels in the blood rise. The condition is well controlled with treatment, but requires accurate diagnosis - it is important to distinguish it from diabetes mellitus and from the habit of drinking a lot.

Short stature in children (stunted growth)
Stunting is the growth of a child that is significantly below the age norm for his gender. More important than the number itself is the growth rate: if a child is consistently short, but gains normally and follows his own curve, the reason is often constitutional and related to the height of his parents. The situation is alarming when the pace slows down and the child begins to lag behind his own previous trajectory. The reasons are very different: familial short stature, constitutional delay in growth and sexual development, chronic diseases, nutritional and absorption disorders, hypothyroidism, growth hormone deficiency and genetic syndromes. Early treatment is important because growth plates close after puberty.

Night sweats
Night sweats are profuse sweating during sleep, which causes you to change your underwear, and not just perspiration in a stuffy room. First, external causes are always excluded: a hot bedroom, a warm synthetic blanket, spicy food and alcohol at night. If sleep conditions are normal, and sweating continues for weeks, look for an internal cause. Most often these are hormonal changes during menopause, increased thyroid function, nightly decreases in sugar in people with diabetes, and taking certain medications. Less commonly, night sweats are a sign of chronic infection, including tuberculosis, or blood diseases, especially if it is combined with weight loss and persistent fever.

Obesity in children
Obesity in children is an excess accumulation of adipose tissue, which is assessed not by appearance, but by body mass index adjusted for age and gender. Adult limits cannot be used in children: the indicator is compared with age standards, since body composition changes as they grow. In the vast majority of cases, the reason is related to lifestyle: sugary drinks, large portions, snacks, little exercise and a lot of screen time. Endocrine and genetic causes are rare and are usually associated with short stature or developmental delay. The problem cannot be postponed “until adolescence”: insulin resistance, fatty hepatosis and high blood pressure develop already in childhood.

Osteopenia
Osteopenia is a condition in which bone mineral density is below normal, but has not yet reached the level of osteoporosis. This is not a disease in the usual sense, but a signal: the bone is losing strength faster than it is being restored, and the risk of fracture is gradually increasing. Osteopenia is asymptomatic, so it is detected either by densitometry or by chance on an x-ray as a loss of bone tissue. It is most often associated with age, vitamin D and calcium deficiency, hormonal changes after menopause, inactivity and certain medications. The good news is that at this stage, bone loss can be slowed and even partially reversed through nutrition, exercise, and correction of deficiencies.

Osteoporosis
Osteoporosis is a disease in which bone density decreases and bones become brittle. It is called a “silent epidemic”: it does not hurt and does not give any symptoms for years, and the first manifestation is often a fracture with minimal trauma - a fall from one’s own height or even an awkward movement. A hip fracture is especially dangerous in the elderly: it dramatically changes the quality of life and is associated with high mortality during the first year. In this case, osteoporosis is detected by a simple test and is well treated.

Primary hyperaldosteronism (Conn's syndrome)
Primary hyperaldosteronism is a condition in which the adrenal glands produce too much aldosterone, regardless of the body's needs. This hormone causes the kidneys to retain sodium and water and excrete potassium, so blood pressure rises and potassium levels in the blood decrease. This was long thought to be rare, but it is now known that it is not uncommon among people with difficult-to-control hypertension. It is important to distinguish it from ordinary hypertension: with unilateral damage to the adrenal gland, surgery can relieve a person of high blood pressure, and with bilateral damage, a special group of drugs helps. In addition, excess aldosterone damages the heart and kidneys more than just high blood pressure.

Increased or decreased TSH
TSH is a thyroid-stimulating hormone of the pituitary gland that controls the functioning of the thyroid gland. The connection between them is reverse: when there is little thyroid hormone, the pituitary gland increases TSH, spurring it on; when there are a lot of hormones, TSH decreases. This is why high TSH usually means decreased gland function, and low TSH usually means increased function. TSH reacts more sensitively and earlier than other indicators, so it is taken as the first test if problems with the thyroid gland are suspected. But the result is influenced by pregnancy, acute illnesses, medications, and even the time of day, so a separate deviating result is a reason to look into it, and not immediately start treatment.

Subacute thyroiditis (de Quervain's thyroiditis)
Subacute thyroiditis, or de Quervain's thyroiditis, is an inflammation of the thyroid gland that usually begins two to six weeks after a viral upper respiratory tract infection. The gland becomes dense and very painful: the pain radiates to the ear, jaw and back of the head, intensifies when swallowing and turning the head, the temperature rises, weakness and aches appear. Damaged cells release a supply of ready-made hormones into the blood, which is why thyrotoxicosis occurs in the first weeks - palpitations, sweating, trembling. Then the supply is depleted, and temporary hypothyroidism occurs. The disease is not purulent and does not require antibiotics; in most people it goes away completely within a few months with the restoration of gland function.
