Diabetes mellitus in children
The most common type of diabetes in children is type 1 diabetes, a disease in which the pancreas' own immune system destroys the beta cells of the pancreas and insulin ceases to be produced. Without insulin, glucose does not enter the cells, accumulates in the blood and is excreted in the urine, carrying water with it. Hence the classic picture: the child begins to drink a lot, urinate frequently and profusely, loses weight with a good appetite, and becomes lethargic. Everything develops over weeks, and sometimes in a matter of days, so it is important not to attribute symptoms to heat or growth. Diabetes is incurable, but with proper treatment, the child grows, learns and plays sports on an equal basis with his peers.

Empty sella syndrome
The sella turcica is a bony depression at the base of the skull in which the pituitary gland lies. They speak of “empty” sella turcica syndrome when the meninges with cerebrospinal fluid protrude into this depression, and the pituitary gland itself is pressed and spread out along the bottom. In the photo, the saddle appears to be filled with fluid, hence the name. It is important to understand: “empty” does not mean the absence of the pituitary gland. In most cases, this is an incidental finding on an MRI done for another reason, and there are no hormonal abnormalities. Treatment is required only when complaints appear or a decrease in pituitary function is confirmed, so the main step after such a conclusion is examination by an endocrinologist.

Kallmann syndrome
Kallmann syndrome is a congenital condition in which puberty does not begin or stops, and the sense of smell is severely reduced or absent. The reason is that in the prenatal period, the nerve cells that produce gonadotropin-releasing hormone do not reach the right place in the brain: they migrate along with the olfactory nerves, and disruption of this pathway affects both hormonal regulation and the sense of smell. As a result, the pituitary gland does not receive a signal, sex hormones are not produced, and the teenager does not enter puberty. The condition responds well to treatment: replacement therapy ensures the development of secondary sexual characteristics, and special regimens allow many patients to have their own children.

Shereshevsky-Turner syndrome
Shereshevsky-Turner syndrome is a chromosomal condition in girls in which one of the two X chromosomes is completely or partially absent. Because of this, the development of the ovaries is disrupted: they are replaced by connective tissue and do not produce enough estrogens. The main manifestations are short stature, absent or incomplete puberty, absence of menstruation and infertility. Often there are structural features of the neck and chest, heart and kidney defects, and hearing loss. Intelligence, as a rule, is preserved, and girls study successfully. The condition is not curable in the sense of eliminating the cause, but timely therapy with growth hormone and sex hormones significantly improves growth, development and quality of life.

Sheehan syndrome (postpartum hypopituitarism)
Sheehan syndrome is pituitary insufficiency that develops after childbirth, complicated by large blood loss and a drop in pressure. During pregnancy, the pituitary gland enlarges and becomes very sensitive to a lack of blood supply, so during massive bleeding, some of its cells die. As a result, the body lacks the hormones that control the thyroid, adrenal glands, ovaries and milk production. The first sign is usually a lack of milk, then menstruation does not return, weakness, chilliness, hair loss, and decreased blood pressure increase. The diagnosis is often made late, although replacement therapy quickly returns the woman to normal health.

Charcot foot (neuroosteoarthropathy)
Charcot's foot is a destruction of the bones and joints of the foot that develops against the background of severe nerve damage, most often with long-term diabetes mellitus. Due to loss of sensitivity, a person does not feel microtrauma and continues to load the foot, and the altered nervous regulation increases blood flow to the bone and accelerates its resorption. The bones gradually break and shift, the arch of the foot collapses, and a characteristic deformity forms. The main danger is that there is almost no pain: the foot simply swells, turns red and becomes hot, and this is often mistaken for inflammation or bruise. Diagnosis at an early stage and complete unloading of the foot allows you to maintain its shape and avoid ulcers and amputation.

Dry mouth (xerostomia)
Xerostomia is a feeling of dry mouth due to lack of saliva. Saliva does more than just moisturize: it washes away food debris, neutralizes acids, contains protective proteins and minerals, and helps form a food bolus and sense taste. When there is not enough of it, multiple caries, gum inflammation, candidiasis and bad breath quickly develop, and wearing removable dentures becomes painful. The most common cause is taking medications: dry mouth is caused by hundreds of medications, from blood pressure medications and diuretics to antihistamines and antidepressants. Less commonly associated with it are diabetes, dehydration, radiation therapy and autoimmune diseases.

Pheochromocytoma
Pheochromocytoma is a rare tumor that grows from cells of the adrenal medulla that produce adrenaline and norepinephrine. The tumor releases these substances into the blood uncontrollably, and the body experiences what happens during severe fright: a sharp rise in blood pressure, palpitations, heavy sweating, headache, pallor, and a feeling of fear. The attack lasts from several minutes to an hour and passes as suddenly as it began. Some people have high blood pressure all the time. A similar tumor outside the adrenal gland is called a paraganglioma. The condition is dangerous due to crises and complications from the heart and blood vessels, but with timely surgery it is most often completely cured.

Endemic goiter and iodine deficiency
Endemic goiter is an enlargement of the thyroid gland in regions where there is little iodine in the soil, water and foods. Iodine is needed for the synthesis of thyroid hormones, and when there is a lack of it, iron begins to grow in order to capture more of the trace element from the blood. At first it increases evenly, then nodes form in it, and some of the nodes eventually begin to work autonomously, causing thyrotoxicosis. Iodine deficiency is especially dangerous in childhood and during pregnancy: thyroid hormones are necessary for the development of the child’s brain. Therefore, the basis for the fight against endemic goiter throughout the world has become iodization of table salt, and the treatment of a particular patient is selected by an endocrinologist.

Endocrine ophthalmopathy (exophthalmos)
Endocrine ophthalmopathy is an autoimmune lesion of the tissues of the orbit, most often associated with diffuse toxic goiter. The same antibodies that stimulate the thyroid gland attack the fiber and extraocular muscles around the eye. The tissues swell and increase in volume, and there is no more space in the bone socket, so the eye moves forward, the eyelids lag behind when looking down, swelling, redness and a feeling of sand appear. The disease goes through a phase of active inflammation and a phase of scarring, and it is in the active phase that treatment is most effective. Smoking is the strongest factor that worsens the course.
