Immunodeficiency (primary and secondary)
Immunodeficiency is a condition in which the body’s defense system cannot cope with its job, and infections are more severe, longer and more frequent than those of others. Primary immunodeficiencies are congenital: the defect is inherent in genes and usually appears in childhood, although some forms are first recognized in adults. Secondary ones are much more common and are associated with external causes - HIV infection, treatment of tumors, long-term use of hormones, severe exhaustion, diabetes, removal of the spleen. It is important to distinguish between real immunodeficiency and common colds: the main thing is not the number of episodes, but their severity, unusual pathogens and poor response to treatment.

Polycythemia vera (erythremia)
Polycythemia vera, or erythremia, is a chronic bone marrow disease from the group of myeloproliferative neoplasms. Due to an acquired mutation, most often in the JAK2 gene, progenitor cells begin to produce red blood cells without an external signal, and the blood becomes thick. Often, leukocytes and platelets increase simultaneously. Thick blood flows worse through the vessels, so the main danger of the disease is thrombosis: heart attack, stroke, thrombosis of deep veins and abdominal vessels. The disease develops slowly, and its first sign is often accidentally noticed high hemoglobin in a routine analysis. With proper treatment, life expectancy can be close to normal.

Leukopenia and neutropenia
Leukopenia is a decrease in the total number of leukocytes in the blood. What is clinically more important is not the total number, but the number of neutrophils: it is these cells that are the first to destroy bacteria and fungi, and their deficiency is called neutropenia. A slight decrease is often temporary and harmless: after a viral infection, while taking certain medications, or as an individual feature. Significant neutropenia increases the risk of severe infections, which can occur with a blurred picture: almost without pus and redness, but with high fever. Therefore, with low leukocytes, the main thing is to understand the cause, assess the depth of the decrease and remember a simple rule: fever against the background of neutropenia requires urgent medical attention.

Lymphoma of the skin (mycosis fungoides)
Mycosis fungoides is the most common T-cell lymphoma of the skin. The name is historical and misleading: the disease has nothing to do with a fungal infection. Its essence is that malignant T-lymphocytes settle in the skin and cause inflammatory changes that are externally indistinguishable from eczema, psoriasis or allergic dermatitis. That is why years often pass from the appearance of the first spots to the diagnosis: a person is treated for a long time by a dermatologist, receives a temporary improvement and returns again with the same complaints. Most patients have a slow-onset form in which the disease is limited to the skin for decades.

Hodgkin's lymphoma (lymphogranulomatosis)
Hodgkin's lymphoma, or lymphogranulomatosis, is a malignant disease of the lymphatic system in which special large Reed-Sternberg tumor cells appear in the lymph node. The disease most often begins with a painless enlargement of the lymph nodes in the neck or above the collarbone and gradually spreads to neighboring groups of nodes, and then to the spleen, lungs and bone marrow. Night sweats, prolonged fever, and weight loss for no reason are also common. The peak incidence occurs in young people and people over 55 years of age. It is important to know the main thing: Hodgkin lymphoma is one of the most curable cancer diseases, and timely treatment allows most patients to fully recover.

Lymphocytosis (increased lymphocytes)
Lymphocytosis is an increase in the number of lymphocytes, cells responsible for antiviral defense and immune memory. In the vast majority of cases, this is a normal reaction of the body to an infection, primarily a viral one, and the indicator returns to normal a few weeks after recovery. It is important to distinguish between relative lymphocytosis, when the proportion of lymphocytes increases due to a decrease in neutrophils, and absolute lymphocytosis, when their number itself increases. A persistent and pronounced increase in adults, especially older adults, requires attention: it may be the first sign of chronic lymphocytic leukemia. A repeat blood test and examination by a doctor will help to figure it out.

Myelodysplastic syndrome
Myelodysplastic syndrome is a group of bone marrow diseases in which hematopoietic cells are present, but they are defective and do not mature properly. The bone marrow may even be overflowing with cells, but few good ones are released into the blood: most die before reaching maturity. The result is paradoxical - rich bone marrow and poor blood. Red blood cells are most often affected, so the disease usually begins with persistent anemia in a person over sixty, which does not respond to iron supplements and vitamins. Over time, white blood cells and platelets may also decrease. Some cases eventually develop into acute myeloid leukemia, so observation is mandatory.

Myelofibrosis
Myelofibrosis is a chronic bone marrow disease from the group of myeloproliferative neoplasms, in which hematopoietic tissue is gradually replaced by connective tissue fibers. The altered cells release substances that stimulate the growth of fibrosis, and the bone marrow loses its ability to produce blood cells. The body tries to establish hematopoiesis in other organs, primarily in the spleen and liver, so the spleen becomes greatly enlarged and causes heaviness in the left hypochondrium and rapid saturation. At the same time, anemia, weakness, night sweats and weight loss increase. The disease can be primary or develop after polycythemia vera or essential thrombocythemia.

Multiple myeloma
Multiple myeloma is a malignant blood disease in which abnormal plasma cells multiply uncontrollably in the bone marrow. Instead of a variety of protective antibodies, they produce the same useless protein - paraprotein. Tumor cells displace normal hematopoiesis and destroy bone from the inside, which is why persistent pain in the back and ribs, low-stress fractures, anemia, high calcium and impaired kidney function are common. Mostly people over 60 years of age are affected. It is still rare to completely cure myeloma, but modern treatment regimens make it possible to achieve long-term remissions and maintain quality of life for years.

Monocytosis (increased monocytes)
Monocytes are large white blood cells that are the first to arrive at the site of inflammation, absorb microbes and dead cells, and then turn into tissue macrophages. In a general blood test, their share usually amounts to several percent of all leukocytes. Monocytosis is an increase in their absolute number. Most often, this is a reflection of the usual process: the body is fighting an infection or completing recovery from an illness. This monocytosis is temporary and does not require treatment. A persistent increase that persists for months, especially together with changes in other blood parameters, enlarged lymph nodes or weight loss, requires consultation with a hematologist and additional examination.
