Thrombocytosis and essential thrombocythemia
Thrombocytosis is an increased number of platelets in a blood test. Most often it is reactive: the bone marrow responds by releasing platelets to inflammation, infection, blood loss, iron deficiency, surgery, or removal of the spleen. Such thrombocytosis goes away along with the cause and in itself is almost not dangerous. Less commonly, we talk about essential thrombocythemia, a chronic bone marrow disease from the myeloproliferative group, in which platelets are produced uncontrollably. It can be asymptomatic for years, but increases the risk of thrombosis and bleeding. It is impossible to distinguish one from the other using one analysis, so if platelets are persistently high, a hematologist is needed.

Enlarged lymph nodes (lymphadenopathy)
Lymphadenopathy is an enlargement of one or more lymph nodes. Lymph nodes work as filters and immune response stations: when an infection appears nearby, they enlarge because immune cells actively multiply in them. Therefore, most often an enlarged node is not a sign of a disease of the node, but of a nearby process: sore throat, caries, skin wounds, viral infection. Such nodes are painful, mobile and decrease within a few weeks. Others are alarming: dense, painless, fused with tissues, growing for more than a month, especially in combination with weight loss and night sweats. In these cases, an examination is needed.

Enlarged spleen (splenomegaly)
Splenomegaly is an enlargement of the spleen, an organ that filters blood, destroys old red blood cells and is involved in immune defense. This is not an independent disease, but a symptom that is always accompanied by a cause: infection, blood stagnation due to liver diseases, blood diseases, autoimmune and metabolic disorders. A slight increase usually does not manifest itself in any way and is detected by chance on an ultrasound. With significant growth, heaviness and pain appear in the left hypochondrium, rapid saturation during meals, and due to increased destruction of blood cells, hemoglobin, leukocytes and platelets decrease. An enlarged spleen is vulnerable to injury and requires care.

Folate deficiency anemia
Folate deficiency anemia occurs when the body does not have enough folic acid, a vitamin B9 essential for cell division and DNA building. The bone marrow suffers most quickly: the precursors of red blood cells do not have time to divide, grow large and immature, and fewer full-fledged cells are released into the blood. Weakness, shortness of breath with normal exertion, pallor, and sometimes a painful bright red tongue appear. Deficiency occurs with a diet poor in vegetables, alcohol abuse, intestinal diseases, as well as with increased need - during pregnancy and rapid growth. It is replenished simply and quickly, but first the doctor must rule out vitamin B12 deficiency, otherwise the treatment may be harmful.

Chronic lymphocytic leukemia
Chronic lymphocytic leukemia is the most common leukemia in older adults. With it, mature-looking, but defective B-lymphocytes accumulate in the bone marrow, blood and lymph nodes. They live too long, crowd out normal cells and do not do their protective work, so the body is less resistant to infections. The disease usually develops slowly and may not cause complaints for decades; It is often found accidentally by persistently elevated lymphocytes in a routine blood test. That is why, in the early stages without symptoms, treatment is often not started, but active observation is chosen - this approach does not worsen the prognosis.

Chronic myeloid leukemia
Chronic myeloid leukemia is a tumor blood disease in which the bone marrow produces an excess of white blood cells and their immature precursors. The reason is known for sure: the exchange of sections between the 9th and 22nd chromosomes creates a shortened chromosome, called the Philadelphia chromosome, and a new gene BCR-ABL. Its protein acts as an always-on switch for cell division. The disease runs quietly for a long time, and the first signal is often a very high number of leukocytes in a blood test along with heaviness in the left hypochondrium due to an enlarged spleen. The emergence of targeted drugs that block this protein has radically changed the prognosis: in most patients, life expectancy is approaching normal.

Eosinophilia (increased eosinophils)
Eosinophilia is an increase in eosinophils, blood cells that are involved in allergic reactions and defense against parasites. Moderate elevations are common and are usually explained by allergies, asthma, atopic dermatitis, or a drug reaction. The second large group of causes are parasitic infestations, especially those in which the larvae migrate through tissues. What matters is not so much the percentage as the absolute number of eosinophils and its persistence: a prolonged pronounced increase can damage the heart, lungs and nervous system, and therefore requires examination. Questioning, tests for parasites and an assessment of the allergic background help to understand the cause.

Erythrocytosis (increased hemoglobin)
Erythrocytosis is an increase in the number of red blood cells, hemoglobin and hematocrit in a blood test. Sometimes this is only an apparent thickening of the blood due to dehydration, but more often the body increases red blood cells in response to a lack of oxygen: with smoking, sleep apnea, chronic diseases of the lungs and heart, living in the mountains. A separate cause is polycythemia vera, a bone marrow disease in which cells multiply uncontrollably. The danger of erythrocytosis is that the blood becomes more viscous and the risk of thrombosis, heart attacks and strokes increases. Therefore, increased hemoglobin in the analysis cannot be ignored: you need to find out what is behind it.
