Cholecystitis: inflammation of the gallbladder - treatment in Tashkent

Other names:

Cholecystitis is inflammation of the gallbladder wall. In the vast majority of cases, it develops against the background of stones: the stone blocks the exit from the bladder, the bile stagnates, and an infection occurs. Acute cholecystitis is a condition requiring hospitalization: without treatment, it can lead to gangrene of the bladder and peritonitis. Chronic cholecystitis is often put too broadly, explaining to them any severity in the right hypochondrium, although in many cases we are talking about functional disorders or other pathology.

With these signs do not wait for a scheduled appointment — the condition requires emergency care.

Acute cholecystitis

  • Constant intense pain in the right hypochondrium, unlike colic, does not go away after a few hours
  • Temperature 37.5–38.5 °C or higher, chills
  • Nausea, vomiting, without relief
  • Increased pain with deep inspiration and with pressure under the right costal arch
  • Rapid pulse, weakness

Treatment is carried out in a hospital: antibiotics, pain relief, infusion therapy. It is considered optimal to perform laparoscopic cholecystectomy in the first days after the onset of an attack - then the operation is technically simpler and there are fewer complications than with delayed intervention.

Chronic cholecystitis

Develops with prolonged existence of stones and repeated episodes of inflammation. It manifests itself less clearly and is often disguised as “liver problems.”

  • Heaviness and dull pain in the right hypochondrium after fatty and fried foods
  • Bitterness in the mouth, especially in the morning
  • Nausea, belching, bloating
  • Intolerance to fatty foods

Survey

Treatment

  • Acute cholecystitis: hospitalization, antibiotics, refusal to eat for a while, infusions, pain relief
  • Laparoscopic cholecystectomy - preferably within the first 72 hours of the onset of the attack
  • In severe condition and high operational risk - percutaneous drainage of the bladder followed by surgery
  • For stones in the common bile duct - endoscopic removal (ERCP) before or after cholecystectomy
  • Chronic calculous cholecystitis - planned cholecystectomy
  • Antispasmodics and painkillers - symptomatically; choleretic drugs for stones are contraindicated

Food

  • Regular split meals 4–5 times a day - stagnation of bile is provoked by long breaks
  • Don't skip breakfast
  • Limit fried, smoked, very fatty, strong broths
  • Adequate fiber and fluid
  • Moderation in alcohol
  • Gradual rather than sudden weight loss for obesity

Frequently asked questions: Cholecystitis

How does colic differ from acute cholecystitis?+
Colic lasts from 30 minutes to several hours and goes away, there is no fever. In acute cholecystitis, the pain persists for more than 6 hours, the temperature rises, and chills appear - this is an indication for hospitalization.
When is the best time to operate?+
In acute cholecystitis, it is optimal in the first 72 hours from the onset of the attack: the operation is technically simpler and there are fewer complications than with delayed intervention.
Is it possible to take choleretic drugs?+
If there are stones - no. They provoke contraction of the bladder and movement of the stone, which leads to blockage of the duct and pancreatitis. The same applies to “tubages”.
Is chronic cholecystitis diagnosed without stones?+
This diagnosis is made too often. If there are no stones, but there are complaints, more often we are talking about a functional disorder, GERD, gastritis or irritable bowel syndrome - and the treatment will be different.
What can you eat?+
The basic rule is regular split meals without long breaks and a mandatory breakfast. Limit fried, smoked and very fatty foods. Strict diets are unnecessary and even harmful.
