Progress of the operation
Through a small incision over the base of the penis (often a V-shaped incision followed by a Y-suturing to lengthen the skin), the surgeon identifies and divides the suspensory ligament at the pubic bone, releasing the inner portion of the corpora cavernosa, which is pushed out. To prevent the ligament from growing back together, fatty tissue is moved into the freed space or a silicone spacer is installed. If there is excess suprapubic fat, liposuction is performed to further increase the visible length. From 2-3 weeks - daily stretching with an extender for 3-6 months; sexual activity after 4–6 weeks.
What does ligamentotomy give?
Part of the penis is hidden under the skin of the pubis and is held in place by the suspensory ligament. Its intersection brings 2–4 cm of the hidden part out - the penis becomes longer at rest and in erection when viewed from above, and the erection angle decreases slightly. The operation is indicated for “hidden” penis, short ligament, obesity with suprapubic fat (in combination with liposuction) and for aesthetic reasons after a frank discussion of expectations: the anatomical length of the cavernous bodies does not change.
Consolidate the result
The main risk is regrowth of the ligament with loss of results, so from 2–3 weeks the patient wears an extender or performs stretching daily for 3–6 months. At the same time, pubic liposuction, skin lengthening with V-Y plastic surgery, and, if desired, penis thickening can be performed. Sexual life - after 4-6 weeks.
