Trichomoniasis: symptoms, tests and treatment in Tashkent

Other names: Трихомоноз, Trichomonas vaginalis, трихомонадная инфекция

Trichomoniasis is caused not by a bacterium or a virus, but by a protozoan - Trichomonas. It is one of the most common curable sexually transmitted infections. Its peculiarity lies in the sharp difference in the picture between men and women: women usually notice profuse foamy discharge and itching, while in men the disease is often almost asymptomatic, which is why they become an invisible source of infection.

With these signs do not wait for a scheduled appointment — the condition requires emergency care.

  • Обильные выделения с резким неприятным запахом и выраженным зудом
  • Боль внизу живота и температура
  • Трихомониаз при беременности
  • Сохранение симптомов после лечения
  • Кровянистые выделения

Features of the pathogen

Trichomonas vaginalis is a single-celled parasite, a motile protozoan that lives in the genitourinary tract. That is why conventional antibacterial drugs do not work on it: a special group of drugs are used for treatment.

Transmission occurs through sexual contact. Trichomonas does not survive outside the body for long, but still longer than many other STI pathogens, so transmission through wet hygiene items is theoretically possible - in practice, this route is extremely rare.

Symptoms

  • In women: copious foamy discharge of a yellowish-greenish color
  • Strong unpleasant odor
  • Severe itching and burning in the external genital area
  • Redness and swelling of the mucous membrane
  • Pain during intercourse
  • Discomfort and pain when urinating
  • Nagging pain in the lower abdomen
  • In men: scanty discharge from the urethra
  • Mild burning sensation when urinating
  • Discomfort after intercourse
  • In men, there is often a complete absence of symptoms
  • Spontaneous disappearance of symptoms in men is possible if the infection persists

Why is it dangerous?

  • In women: pelvic inflammatory disease
  • Pregnancy complications, premature birth, low birth weight
  • Increased risk of contracting HIV and other infections due to damage to the mucosa
  • In men: urethritis, prostatitis, epididymitis
  • Possible deterioration of sperm parameters
  • Chronic course with periodic exacerbations
  • Disturbance of the normal vaginal microflora with the addition of bacterial vaginosis

Diagnostics

  • PCR is the most sensitive method, especially important for men in whom the pathogen is more difficult to detect
  • Microscopy of a native smear - allows you to see mobile Trichomonas, but is informative only with a quick study of fresh material
  • Stained smear microscopy - less sensitive
  • Sowing on special media
  • In men, urethral discharge, the first portion of urine, and sometimes prostate secretion are examined.
  • Screening for other STIs - the combination is common
  • Partner examination

Please note: a negative microscopy result in a man does not rule out infection. That is why, in case of trichomoniasis in a partner, a man is examined using the PCR method, and is often treated even if the result is negative.

Treatment

  • Preparations of the nitroimidazoles group are specific agents that act on protozoa; The doctor determines the regimen and dose
  • Conventional antibiotics are ineffective for this infection
  • Mandatory simultaneous treatment of all sexual partners, even if their tests are negative
  • Complete abstinence from alcohol during treatment and for several days after - combination with these drugs causes a severe reaction with nausea, vomiting, palpitations
  • Sexual rest until both partners complete treatment
  • Local medications as an adjunct, but not as the basis of treatment
  • Control examination at the time prescribed by the doctor
  • If the first course is ineffective, another scheme is used, sometimes longer
  • Special regimens during pregnancy - prescribed by a doctor taking into account the period
  • Restoration of vaginal microflora after treatment in women

Why treatment sometimes doesn't work

Frequently asked questions: Trichomoniasis

Why does my husband’s test come back negative, but I have trichomoniasis?+
In men, trichomonas are much more difficult to detect: there are fewer of them, and conventional microscopy often does not detect them. A negative result does not rule out infection. Therefore, when trichomoniasis is detected in a woman, the partner is recommended to undergo PCR testing and, as a rule, treatment regardless of the result - otherwise re-infection is inevitable.
Is it true that you should not drink alcohol during treatment?+
Yes, and this is not a formality. Drugs used for trichomoniasis, when combined with alcohol, cause a pronounced reaction: nausea, vomiting, facial flushing, palpitations, drop in blood pressure. You need to abstain from alcohol throughout the entire course and for a few more days after its completion.
Do suppositories and topical remedies help?+
As an independent treatment - no. Trichomonas can be found not only in the vagina, but also in the urethra, paraurethral glands, and in men, in the prostate, where local drugs do not penetrate. The basis of treatment is a systemic drug, and local agents can only be used as an adjunct.
Can you get infected in the pool or through a towel?+
Almost none. Although Trichomonas survives in a humid environment longer than many other pathogens, direct contact with a sufficient amount of the pathogen is required for infection. The real route of transmission is sexual. The swimming pool version is usually used as an explanation rather than reflecting reality.
Can trichomoniasis go away on its own?+
In men, symptoms often disappear on their own, but this is not recovery: the pathogen remains, and the person continues to be a source of infection. In women without treatment, the infection usually takes a chronic course with exacerbations. Spontaneous healing is a rare exception that you should not count on.
Does it affect pregnancy?+
Yes, trichomoniasis during pregnancy is associated with an increased risk of premature birth and low birth weight of the newborn. Treatment is carried out, but the doctor selects the drugs and regimen taking into account the duration of pregnancy. Self-medication during this period is unacceptable.
