Systemic scleroderma
Systemic scleroderma is a rare autoimmune connective tissue disease that causes excessive collagen production and damage to small blood vessels. The skin gradually thickens and loses mobility, starting with the fingers and face, and vascular disorders are manifested by Raynaud's syndrome: in the cold or with excitement, the fingers turn white, then turn blue and red, while they hurt and go numb. The danger of the disease is not in the skin, but in the internal organs: the esophagus, lungs, heart and kidneys can be affected. Early diagnosis and observation by a rheumatologist make it possible to identify these changes in time and prescribe treatment that significantly changes the course of the disease.

Systemic vasculitis
Systemic vasculitis is a group of diseases in which the immune system attacks the wall of its own blood vessels. The inflamed vessel swells, its lumen narrows or is closed by a blood clot, and the tissues it nourishes lack oxygen. Since the vessels are everywhere, the manifestations are extremely varied: prolonged fever without obvious infection, weight loss, rash, pain in the joints and muscles, damage to the kidneys, lungs, nerves or eyes. Vasculitis is uncommon, but without timely treatment it can quickly damage vital organs, so early diagnosis is fundamentally important.

Trochanteritis (bursitis of the hip joint)
Trochanteritis is pain and inflammation in the greater trochanter, a bony protrusion on the outer surface of the thigh. Previously, the condition was considered only an inflammation of the bursa; today they more often talk about damage to the tendons of the gluteus medius and minimus muscles, which are attached to this trochanter. A typical complaint is pain on the side of the thigh, which prevents lying on that side, worsens when walking, climbing stairs and getting up from a chair. The pain may radiate along the outer surface of the thigh to the knee, but does not go below the knee. The condition is often mistaken for arthrosis or disc herniation, although it is treated differently.

Uveitis and iridocyclitis
Uveitis is an inflammation of the choroid of the eye, which nourishes the retina and internal structures. When inflammation affects the iris and ciliary body, they speak of iritis and iridocyclitis, or anterior uveitis: this is the most common form. The eye turns red around the cornea, pain, photophobia and fog appear before the eye. The reasons are different - autoimmune and rheumatic diseases, infections, injuries, but often the cause cannot be established. The main thing that the patient should know is that uveitis cannot be treated with ordinary drops for redness and requires a quick examination by an ophthalmologist, because without treatment it can lead to permanent vision loss.

Erythema nodosum
Erythema nodosum is an inflammation of the subcutaneous fatty tissue, which manifests itself as painful red nodules, most often on the front surface of the legs. The nodes are dense, hot to the touch, and difficult to clearly define with your fingers; they never open or fester, but after one or two weeks they change color, like a bruise, and gradually dissolve. This is not an independent disease, but the body’s reaction to an infection, medication or systemic disease, so the doctor’s main task is to find the trigger factor. Often this is a previous streptococcal infection, but tuberculosis and sarcoidosis must be excluded.

Morning joint stiffness
Morning stiffness is a feeling of joint stiffness after sleep or prolonged rest, when the joints seem to be full and difficult to develop. It is not a disease in itself, but it is one of the most informative symptoms in rheumatology, because its duration helps to distinguish between the two large types of joint diseases. Brief stiffness of up to 20–30 minutes, which quickly disappears with movement, is typical of osteoarthritis and overuse. Stiffness for more than 30–60 minutes, especially in the small joints of the hands and symmetrically, indicates inflammatory arthritis, and with stiffness in the lower back, an inflammatory lesion of the spine. This is a reason not to wait, but to get examined.

Crunching in joints
Crunching, clicking and cracking in the joints is a very common complaint, and most often it is harmless. The main reason for the painless sound is cavitation: gas bubbles in the joint fluid collapse when the pressure changes. This crunch does not damage the cartilage and does not lead to arthrosis. The sound can also occur when a tendon or ligament slips over a bony protrusion, or when there is increased joint mobility. It is not the sounds themselves that should be alarming, but what accompanies them: pain, swelling, stiffness in the morning, limitation of movements or a feeling of blockage. These are already signs of damage to the cartilage, meniscus or inflammation, and here a doctor’s examination is needed.

Episcleritis and scleritis
The white of the eye is covered on the outside with a thin transparent membrane - the episclera, and under it lies the dense white sclera, the frame of the eyeball. Inflammation of the superficial layer is called episcleritis: it gives a limited pink or bright red spot, mild discomfort and almost always goes away on its own. Scleritis is an inflammation of the deep membrane, occurs less frequently, occurs with severe aching pain radiating to the temple and jaw, and can lead to thinning of the sclera and loss of vision. Both conditions are often associated with systemic rheumatic diseases, so it is important not just to drop drops, but to understand the cause.

Juvenile idiopathic arthritis
Juvenile idiopathic arthritis is a chronic inflammation of the joints that begins before age 16 and lasts for at least six weeks. This is not a “childhood form of rheumatism” or a consequence of a cold: for some unknown reason, the immune system begins to attack its own joint membrane. The joint swells, becomes warm, the child spares his leg or arm, moves stiffly in the morning and walks only after a while. Small children do not directly complain of pain, but begin to limp, ask to be held, or stop using their hand. A separate danger of the disease is asymptomatic inflammation of the eye, which can only be detected during an examination by an ophthalmologist, so vision tests are mandatory.

Ankylosing spondylitis (Bechterew's disease)
Ankylosing spondylitis is a chronic inflammation of the joints of the spine and sacroiliac joints, which, without treatment, gradually limits the mobility of the back. It begins early, more often in men between the ages of twenty and forty, and almost always with the same complaint - lower back pain. This is why it is not recognized for so long: back pain is usually explained by osteochondrosis, sedentary work or lifting heavy objects. Meanwhile, this pain can be distinguished from ordinary pain already at the first appointment, because it behaves in the opposite way: it intensifies at rest and in the second half of the night, wakes a person up in the morning, is accompanied by many hours of stiffness and decreases not with rest, but with movement. From the first symptoms to diagnosis, on average, it takes from five to seven years - and this, in fact, is the main problem of the disease, because it is during these years that treatment would be most effective.
