Hip pain
The hip joint bears the main load when walking, so pain in it quickly changes the way of life: it becomes difficult to put on socks, get up from a chair, or walk the usual distance. It is important to understand that “hip pain” can come from the joint itself, from the surrounding tendons, or even from the lower back. True joint pain is most often felt in the groin and can radiate to the knee, pain on the side of the thigh is usually associated with the tendons and trochanteric bursa, and pain in the buttock with a shooting in the leg is associated with the spine. Both examination and treatment depend on where exactly it hurts.

Bursitis
Bursitis is an inflammation of the bursa, a small sac of fluid that cushions friction between bone, tendon and skin. These bags are located at the elbow, knee, shoulder, hip and heel. With constant pressure, bruise, gout or infection, the bursa fills with fluid and a soft, painful swelling appears over the joint. Movement in the joint itself is usually preserved, but becomes unpleasant. Most bursitis goes away within a few weeks after the load is removed, but it is important to distinguish regular inflammation from purulent inflammation in time: it requires an urgent opening and antibiotics. Recurrent bursitis requires a search for the cause, not just pain relief.

Hemorrhagic vasculitis (Henoch-Schönlein purpura)
Hemorrhagic vasculitis, or Henoch-Schönlein purpura, is an inflammation of the smallest vessels, in which immune complexes with immunoglobulin A are deposited in their walls. Children of preschool and primary school age are most often affected, usually one to three weeks after a respiratory infection. Typical manifestations are a symmetrical rash on the legs and buttocks that does not fade with pressure, joint pain, cramping abdominal pain and kidney damage. In most children, the disease goes away within a few weeks, but the condition of the kidneys should be monitored for several more months after recovery.

Joint hypermobility (connective tissue dysplasia)
Joint hypermobility is a range of movements that is noticeably greater than normal: elbows and knees hyperextend, the thumb is easily pressed against the forearm, a person does the splits without warming up. This in itself is not a disease, but a feature of the structure of connective tissue, and for many people it does not cause any problems. The syndrome is said to occur when excessive mobility is accompanied by complaints: pain in the joints and back in the evening, frequent subluxations and sprains, flat feet, fatigue, and a tendency to bruise. There is almost no need for medications here - the basis of assistance is in well-chosen exercises that strengthen the muscles around the joints.

Osteoporosis
Osteoporosis is a disease in which bone density decreases and bones become brittle. It is called a “silent epidemic”: it does not hurt and does not give any symptoms for years, and the first manifestation is often a fracture with minimal trauma - a fall from one’s own height or even an awkward movement. A hip fracture is especially dangerous in the elderly: it dramatically changes the quality of life and is associated with high mortality during the first year. In this case, osteoporosis is detected by a simple test and is well treated.

Humeroscapular periarthritis
Humeral periarthritis is a collective name for painful conditions of the soft tissues around the shoulder joint. Most often, this diagnosis hides damage to the rotator cuff tendons and adhesive capsulitis, in which the joint capsule becomes inflamed, thickens and tightens. A person first notices nighttime shoulder pain, then finds that he cannot put his arm behind his back, comb his hair or take off his shirt. The joint itself is not destroyed: the tendons, bursae and capsule are affected. The condition lasts for months, but with proper rehabilitation, hand mobility is restored in most cases.

Increased uric acid (hyperuricemia)
Hyperuricemia is an increase in the level of uric acid in the blood. It is formed during the breakdown of purines, which come from food and are constantly formed in the body itself, and are excreted mainly by the kidneys. The increase is often associated not with excess nutrition, but with insufficient excretion by the kidneys. By itself, it usually does not cause complaints and is often discovered by chance. What matters is that with prolonged excess, uric acid salts can be deposited in the joints, causing gout, and in the kidneys, forming stones. At the same time, not all hyperuricemia requires medication: for many people, observation and correction of risk factors is sufficient.

Psoriatic arthritis
Psoriatic arthritis is a chronic inflammatory joint disease that develops in some people with psoriasis. The immune system attacks not only the skin, but also joints, tendon attachments to bones, the spine and nails. Most often, skin psoriasis appears first, and joint complaints appear years later, but in some patients the sequence is reversed. The severity of skin manifestations does not determine the severity of arthritis: extensive plaques can be combined with mild joint damage and vice versa. The disease can cause permanent destruction of joints, so early contact with a rheumatologist and timely initiation of therapy are fundamentally important.

Reactive arthritis (Reiter's syndrome)
Reactive arthritis is an inflammation of the joints that occurs several weeks after an intestinal or urogenital infection. There is no pathogen in the joint itself: the immune system, having responded to the microbe, continues to attack its own tissues due to the similarity of their structures. Typically, a few joints of the legs are affected - the knee, ankle, and toe joints, often asymmetrically. The triad with inflammation of the joints, eyes and urethra is considered classic, but not everyone has it completely. Most patients recover within a few months, but in some the disease takes a protracted course, so observation by a rheumatologist is necessary.

Rheumatism (acute rheumatic fever)
Acute rheumatic fever, which is commonly called rheumatism, is an inflammatory disease that develops 2-4 weeks after a sore throat or pharyngitis caused by group A beta-hemolytic streptococcus. The immune system, attacking streptococcus, mistakenly damages its own tissues with a similar structure: the heart, joints, nervous system, skin. Children and adolescents aged 5 to 15 years are most often affected. The main danger is heart damage (rheumatic carditis), which can lead to the formation of chronic rheumatic heart disease with valve defects. Joint pain goes away without a trace, and valve changes remain for life. Timely treatment of streptococcal sore throat with antibiotics and prevention of repeated attacks can prevent these consequences.
