Watery eyes (epiphora)
Epiphora is excessive lacrimation, in which the tear does not go into the nose, but overflows the edge of the eyelid. There are two groups of reasons: too much tears are produced or they do not drain well. Paradoxically, one of the most common causes is dry eye: insufficient quality of the tear film irritates the cornea, and the gland reflexively throws out a portion of tears. The second large group is narrowing or blockage of the lacrimal ducts, inflammation of the lacrimal sac, and incorrect position of the eyelids. Watery eyes are rarely dangerous in themselves, but they make it difficult to read, drive, or work, and sometimes indicate a problem that would be better addressed sooner.

Spasm of accommodation (false myopia)
Accommodation spasm is a persistent tension in the ciliary muscle of the eye, which is responsible for focusing near. When working with books, notebooks, phones and computers for a long time, the muscle stops completely relaxing, and the eye seems to get stuck in near vision mode. As a result, distance vision deteriorates, fatigue and headaches appear, and a routine test may show myopia, which in fact is not there. Therefore, the condition is called false myopia. Most often it occurs in schoolchildren and students, but it also occurs in adults with intense visual load. The spasm is reversible, but without eye rest and treatment it can contribute to the development of true myopia. An accurate diagnosis is made only after a vision test with pupil dilation.

Dark circles under the eyes
Dark circles under the eyes are darkening of the skin and shadows in the lower eyelids, which make the face look tired, regardless of how you actually feel. The skin here is the thinnest on the body, there is almost no subcutaneous tissue, so the blood vessels and the orbicularis oculi muscle are visible through it. The causes are divided into several types: vascular, when a bluish tint is visible, pigmentary with a brown tone, structural, associated with the shape of the orbit and hernias of the eyelids, and mixed. Lack of sleep, dehydration, smoking, allergies, eye rubbing, heredity and iron deficiency all contribute. The tactics also depend on the type: some are helped by the regimen and treatment of anemia, others by procedures, and others by volume correction.

Trauma (contusion) of the eye
Contusion is damage to the eye due to a blunt blow: from a ball, fist, elbow, traffic jam, branch, fall or accident. At the moment of impact, the eyeball contracts from front to back and expands to the sides, so not only the eyelids are affected, but also the internal structures: the iris, lens, retina, optic nerve. That is why the external picture is deceptive: a bruise and redness may be the only thing visible, while inside there is already a hemorrhage, a tear in the retina or a displacement of the lens. Some complications, such as glaucoma, appear months and years later. Therefore, after any serious blow, an examination of the fundus is necessary.

Trachoma
Trachoma is a chronic eye infection caused by certain types of chlamydia. It is transmitted through close contact: through hands, shared towels and bed linen, as well as by flies landing on eye discharge. The disease begins as protracted conjunctivitis with granularity of the inner surface of the upper eyelid. Repeated infections over many years lead to scarring that causes the eyelid to roll inward and the eyelashes to rub against the cornea. Permanent injury to the cornea causes clouding and irreversible loss of vision. This is why trachoma remains the leading infectious cause of blindness in the world. However, in the early stages it responds well to antibiotic treatment.

Uveitis and iridocyclitis
Uveitis is an inflammation of the choroid of the eye, which nourishes the retina and internal structures. When inflammation affects the iris and ciliary body, they speak of iritis and iridocyclitis, or anterior uveitis: this is the most common form. The eye turns red around the cornea, pain, photophobia and fog appear before the eye. The reasons are different - autoimmune and rheumatic diseases, infections, injuries, but often the cause cannot be established. The main thing that the patient should know is that uveitis cannot be treated with ordinary drops for redness and requires a quick examination by an ophthalmologist, because without treatment it can lead to permanent vision loss.

Chorioretinitis (posterior uveitis)
Chorioretinitis is an inflammation of the choroid of the eye and the adjacent retina. The choroid nourishes the outer layers of the retina, so inflammation quickly involves both structures. Pain and redness of the eye are often absent: the posterior parts of the eye do not have pain receptors. A person notices floating clouds, fog, a dark spot in the field of vision, or distortion of straight lines. The cause is infection, primarily toxoplasmosis, as well as autoimmune and systemic diseases. The danger is that the inflammation leaves scars on the retina, and if the lesion is in the central zone, vision is irreversibly impaired.

Central serous chorioretinopathy
Central serous chorioretinopathy is a condition in which clear fluid accumulates under the central part of the retina and rises in a bubble, like peeling wallpaper. The reason is the increased permeability of the choroid and the inability of the pigment layer to pump fluid back. A person sees a blurry or dark spot in the center, objects seem smaller and a little yellower, straight lines are curved. Men of working age are more likely to get sick, and there is a connection with a busy lifestyle and taking hormonal drugs. For most, the fluid resolves on its own within a few months, but a prolonged course requires treatment.

Endocrine ophthalmopathy (exophthalmos)
Endocrine ophthalmopathy is an autoimmune lesion of the tissues of the orbit, most often associated with diffuse toxic goiter. The same antibodies that stimulate the thyroid gland attack the fiber and extraocular muscles around the eye. The tissues swell and increase in volume, and there is no more space in the bone socket, so the eye moves forward, the eyelids lag behind when looking down, swelling, redness and a feeling of sand appear. The disease goes through a phase of active inflammation and a phase of scarring, and it is in the active phase that treatment is most effective. Smoking is the strongest factor that worsens the course.

Endophthalmitis
Endophthalmitis is a severe inflammation of the inner membranes and cavities of the eye, most often caused by bacteria or fungi. The pathogen gets inside during a penetrating injury, after surgery or intraocular injection, or less often through the bloodstream from another source of infection. There are almost no barriers inside the eye to hold back microbes, so inflammation develops rapidly: within hours, vision can drop to light perception. Typical signs are rapidly increasing pain, redness, blurred vision, and pus in the anterior chamber. This is a condition in which the hours are counted: the earlier treatment is started, the higher the chance of maintaining vision. With such symptoms, you should immediately seek ophthalmological help.
