Reasons

  • Herpes simplex virus is the most common cause of sporadic encephalitis; affects predominantly the temporal lobes

  • Complication of measles, rubella, mumps - preventable by vaccination

Symptoms

  1. Начало: лихорадка, головная боль, общее недомогание — картина похожа на грипп.
  2. Через часы или дни присоединяются мозговые симптомы: изменение поведения, раздражительность, неадекватность, галлюцинации, нарушение речи и памяти.
  3. Судороги — частый и тревожный признак, особенно первый в жизни припадок у взрослого.
  4. Очаговые симптомы: слабость в конечностях, асимметрия лица, нарушение координации.
  5. Угнетение сознания: от сонливости до комы при тяжёлом течении.
Родственники нередко описывают начало как «он стал сам не свой»: странные высказывания, немотивированные поступки, потеря ориентации. При лихорадке это не «нервы» и не переутомление, а повод для экстренного обращения.

Diagnostics

  1. МРТ головного мозга — метод выбора; при герпетическом энцефалите выявляет характерное поражение височных долей.
  2. Люмбальная пункция с исследованием ликвора и ПЦР на вирусы — подтверждает возбудителя.
  3. ЭЭГ — выявляет характерные изменения и эпилептическую активность.
  4. Анализы крови, серология, при подозрении на аутоиммунную форму — исследование специфических антител.
  5. КТ — если МРТ недоступна или требуется срочно исключить кровоизлияние.

Treatment

Key principle: if herpetic encephalitis is suspected, antiviral therapy is started immediately, without waiting for PCR results. A delay of one day significantly worsens the outcome, and the drug itself, in the absence of confirmation, will be canceled without harm.
