Swelling or fatty hernia
This is the key question on which everything further depends. Swelling appears or intensifies in the morning, after salty foods and alcohol, by the middle of the day it noticeably decreases, the skin is taut and slightly shiny. A fatty hernia appears as a permanent ridge under the lower eyelid that does not change throughout the day and becomes more noticeable when looking up or while sitting. A combination often occurs: swelling periodically appears against the background of hernias. A simple self-check is to compare photographs in the morning and in the evening, but the final conclusion is made by the doctor during the examination.

Causes of edema
Most often this is a lifestyle: excess salt and liquid at night, alcohol, lack of sleep, working behind a screen without breaks, sleeping on your stomach. Allergic rhinitis and conjunctivitis cause swelling of the eyelids with itching and redness. Fluid retention is caused by hormonal fluctuations before menstruation, pregnancy and certain medications. Systemic causes are most important not to miss: kidney disease is manifested by morning swelling of the face and changes in urine analysis, hypothyroidism - dense swelling, dry skin and chilliness, heart failure - swelling that increases in the evening and begins in the legs.

Why do fatty hernias appear?
With age, the connective tissue septum that holds the fat bags around the eye weakens, and the ligaments of the middle third of the face stretch. The fat moves forward and down, the skin loses its elasticity, and a characteristic ridge with a groove underneath is formed. Heredity plays a significant role: in some people, hernias are noticeable already at 25–30 years of age. The process is enhanced by smoking, chronic lack of sleep, significant weight fluctuations and constant rubbing of the eyelids. Creams, patches and massage do not affect the volume of fat bags - they can only reduce the accompanying swelling.

When is an examination needed?
If the swelling has appeared recently, is persistent, or is combined with other complaints, it is reasonable to exclude internal causes. The basic minimum is a general urine test and assessment of kidney function, TSH, blood pressure measurement, and, if necessary, assessment of heart function. Unilateral swelling with pain, redness and fever requires urgent examination: this may be inflammation of the tissue of the orbit. Protrusion of the eyes, double vision, sweating and palpitations indicate a disease of the thyroid gland with damage to the orbit, which is treated by an endocrinologist and ophthalmologist.

What really helps
For edema, simple measures work: limit salt and alcohol, do not drink much at night, sleep for 7-8 hours on your back with your head slightly elevated, take breaks from working behind the screen, use a light cooling treatment and gently remove makeup. The effect appears in weeks, not in one morning. Diuretics “for the face” cannot be taken: they do not eliminate the cause and are dangerous. For fatty hernias, the only reliable method is lower blepharoplasty, in which fat is removed or redistributed. If the problem is a deep furrow and loss of cheek volume, hyaluronic acid fillers help.
