Low blood pressure: when hypotension is safe and when it requires examination - consultation in Tashkent

Other names:

Pressure below 90/60 is considered low, but this figure in itself is not a diagnosis. The purpose of blood pressure is to supply blood to the brain, heart and kidneys, and if a person with indicators of 95 to 60 is alert, productive and does not experience dizziness, his blood pressure is doing its job, which means there is nothing to treat. Moreover, persistently low blood pressure in young, thin people is associated with a lower risk of cardiovascular disease in the future. Another distinction is fundamentally important - between pressure that has always been low and pressure that has recently decreased. The first is almost always harmless. The second is a symptom, which could be due to blood loss, a side effect of medications, adrenal or thyroid dysfunction, infection, or a heart problem. A separate and most practically significant situation is a drop in pressure when standing up in older people, because it leads to falls and fractures.

With these signs do not wait for a scheduled appointment — the condition requires emergency care.

What pressure is considered low and why the number is not yet a diagnosis

The formal boundary is 90 to 60, but it cannot be applied mechanically. The blood pressure of the same person fluctuates throughout the day, depending on the air temperature, time of the last meal, body position, quality of sleep, and even on how correctly the cuff is applied. Therefore, a single low measurement does not mean anything; What matters is repetition and, most importantly, well-being.

  • They evaluate not an isolated number, but a combination of numbers and symptoms: dizziness, weakness, darkening of the eyes
  • It is important what the pressure was before: a decrease from 140 to 110 in a hypertensive patient may feel worse than a stable 95 in a young woman
  • A cuff that does not fit, talking during measurement, crossed legs and an unsupported arm will distort the result.
  • You need to measure after five minutes of rest, twice with an interval of one or two minutes, and write down both results
  • At the first visit, the pressure is measured on both hands: a persistent large difference between them requires a separate examination
  • Pulse pressure is also important - the difference between the upper and lower readings
  • A home blood pressure monitor should be a brachial one; wrist models are much more likely to give erroneous results

Hence the practical conclusion for the first visit. Instead of complaining “I have low blood pressure,” it is much more useful to bring a diary for one or two weeks: morning and evening, numbers, well-being, notes on taking medications. Such a table gives the doctor something that a one-time measurement in the office will not provide - a picture of real fluctuations and their connection with complaints.

Constitutional hypotension: when there is nothing to treat

The most common variant of low pressure is the one that accompanies a person throughout his entire adult life. Usually these are young people, often women, of thin build, with blood pressure in the region of 90–100 over 55–65. They may notice chilliness, a tendency to motion sickness, poor tolerance of stuffiness and heat, slight dizziness when standing up suddenly, but at the same time they are fully functional and do not have any abnormalities during examination.

  • Blood pressure has been low since youth and has not changed abruptly
  • Parents or other relatives are also hypotensive
  • I feel generally good, I have the strength to work and be physically active.
  • Stuffiness, prolonged standing, hot baths and saunas, severe heat are poorly tolerated
  • Possible tendency to get motion sickness in transport
  • The examination does not reveal anemia, disorders of the thyroid gland, adrenal glands or heart
  • During physical activity, blood pressure rises adequately rather than falling.

This condition does not require treatment, and trying to “raise blood pressure” with drugs does more harm than good. What really helps is drinking enough fluids, eating regularly without long breaks, aerobic exercise, the habit of getting up smoothly, and avoiding standing still for long periods of time. Some of the complaints that are associated with low blood pressure - headache, fatigue, drowsiness - are actually caused by lack of sleep, iron deficiency or dehydration.

Sudden decrease in blood pressure: what to exclude

A completely different situation arises when the pressure has recently decreased in a person who previously had normal or high blood pressure. Here, low blood pressure is not a feature, but a symptom, and the doctor’s task is to find the cause. The list of what needs to be excluded is quite clear, and most of it is verified by simple and accessible research.

A review of medications taken brings results more often than any complex research. An elderly person often takes medications prescribed by different doctors at different times, and some of them reduce blood pressure. Added to this is a pattern: with age and with weight loss, the need for a dose of antihypertensive drugs decreases, and the regimen that was suitable five years ago gives an excessive effect. Therefore, it makes sense to bring the entire package, including supplements, to the doctor.

Orthostatic hypotension and falls in the elderly

Orthostatic hypotension is a drop in blood pressure when moving from a lying or sitting position to a standing position. Normally, when standing up, the blood vessels reflexively constrict, and the brain continues to receive enough blood. With age, this reaction slows down, and medications, dehydration and bed rest weaken it even more. The result is darkening of the eyes, dizziness, unsteadiness, and often a fall with a fracture. It is a hip fracture after such a fall that often becomes an event that dramatically changes the life of an elderly person, so the importance of this topic cannot be overestimated.

  • The diagnosis is simple: blood pressure is measured while lying down, then one and three minutes after standing up
  • Symptoms usually occur in the first seconds or minutes after rising, especially in the morning
  • Provoke fever, hot bath, rich food, alcohol, prolonged bed rest
  • A separate option is a decrease in blood pressure after eating, which is typical for the elderly: weakness and drowsiness 30–60 minutes after a heavy meal
  • You need to get up in two stages: first sit on the edge of the bed, sit for half a minute or a minute, then stand up, holding onto a support
  • Compression stockings and calf exercises before lifting reduce symptoms
  • Raising the head of the bed by 10–15 centimeters helps with severe orthostatic hypotension
  • Drinking enough throughout the day and a glass of water immediately after waking up is a simple and effective measure.

The situation at home deserves special attention: the severity of the consequences is determined not so much by dizziness, but by what a person hits when falling. Remove sliding mats and wires, install night lighting on the way to the toilet, install grab bars in the bathroom, replace slippers with stable shoes - these measures are inexpensive and reduce the risk of fracture more effectively than any drugs. It may also be helpful to have your vision tested and to review sleeping pills that may impair your balance at night.

Coffee, salt and other tips: where is their limit?

Popular recommendations for low blood pressure are well known: strong coffee, salty, sweet tea. Some of them actually make sense, but each has a clear limit of applicability, and this is what is usually not talked about. It’s worth examining them separately, because uncontrolled adherence to these tips creates new problems.

  • Caffeine briefly increases blood pressure, but the effect is short-lived, and with regular use it becomes addictive. In addition, coffee increases fluid excretion, provokes heart palpitations and disrupts sleep.
  • Salt increases blood pressure due to fluid retention, and with constitutional hypotension, a moderate increase in blood pressure is acceptable. But the advice does not apply to heart failure, kidney disease and the elderly with blood pressure fluctuations
  • Drinking enough fluids is the safest and often most effective measure: the volume of circulating blood directly affects blood pressure
  • A quickly drunk glass of cool water can significantly increase blood pressure for a short time and helps with orthostatic episodes
  • Eating small meals prevents post-lunch low blood pressure better than any medications
  • Regular aerobic exercise and leg muscle exercises improve vascular reactions; complete rest, on the contrary, worsens the tolerance of a vertical position
  • Alcohol dilates blood vessels and lowers blood pressure - with hypotension, it worsens the condition, not helps
  • Hot baths, saunas and prolonged exposure to heat provoke episodes and should be avoided

Medicines to increase blood pressure exist, but they are used according to strict indications - for severe, confirmed orthostatic hypotension, for neurological diseases with impaired autonomic regulation, for adrenal insufficiency. They are prescribed by a doctor, and almost always after non-drug measures have been exhausted and the medications taken have been reviewed. Self-administration of “toning” medications, tinctures and stimulants for an unknown cause of low blood pressure is doubly dangerous: it provides a temporary improvement in well-being and at the same time delays the search for the real cause, which may turn out to be serious.

Heat, pregnancy and everyday situations

In hot climates, hypotension behaves differently than in temperate climates. Dilation of skin blood vessels for heat transfer, loss of fluid and salts through sweat, loss of appetite - all this naturally reduces blood pressure, and in the summer complaints intensify even among those who feel normal the rest of the year. This also applies to people taking medications for hypertension: their usual dose in the heat is often excessive, and it is in the summer that episodes of dizziness and falls occur more often.

In pregnant women, a decrease in blood pressure in the second trimester is a common physiological phenomenon, and in itself it does not cause alarm. It has another meaning: in the second half of pregnancy, lying on your back for a long time leads to compression of a large vein by the enlarged uterus, the pressure drops sharply, weakness and dizziness appear. The solution is simple - lie on your side, preferably on your left. At the same time, fainting and severe weakness in a pregnant woman require examination, and not an explanation “this is normal”: behind them there is often anemia, which responds well to treatment.

Frequently asked questions: Arterial hypotension (low blood pressure)

Is blood pressure 90/60 a disease?+
By itself, no. If such numbers accompany a person from his youth, he is able to work and does not experience dizziness or fainting, we are talking about a constitutional feature that does not require treatment. Diagnosis and examination are needed in a different situation: when the pressure has recently decreased in a person who previously had normal or high blood pressure, or when low numbers are accompanied by severe symptoms.
Why do your eyes get dark when you suddenly stand up?+
When you stand up, blood flows downward under the influence of gravity, and the vessels must reflexively narrow so that the brain continues to receive enough blood. If this reaction is delayed, a short-term decrease in blood flow occurs - hence darkening of the eyes and dizziness. Helps you get up in two stages: first sit on the edge of the bed for half a minute, then rise, holding onto a support.
Is it possible to drink coffee to increase blood pressure?+
In moderation, yes, caffeine increases blood pressure in the short term. But you shouldn’t rely on it as a treatment: the effect is short-lived, with regular use it becomes addictive, and coffee increases fluid excretion, provokes palpitations and impairs sleep. Drinking enough fluids, regularly eating small portions, and physical activity work much more reliably.
Is it good to eat more salt if you have low blood pressure?+
With constitutional hypotension in a young healthy person, a moderate increase in salt intake is acceptable. But this advice cannot be used for heart failure, kidney disease and in older people with blood pressure fluctuations - excess salt is harmful for them. Therefore, the issue of salt is resolved individually with a doctor, and not according to the general rule from the Internet.
In what cases is low blood pressure dangerous?+
The danger is created by a sudden decrease and accompanying symptoms: fainting with injury or during exercise, chest pain, shortness of breath, rapid weak pulse, black stools or vomiting with blood, high temperature with confusion, a sharp drop in blood pressure after taking a new drug. All these situations require urgent treatment, rather than independent measurements and observation.
An elderly relative feels dizzy when standing up - what to do?+
First of all, measure the pressure while lying down and one and three minutes after standing up and show these numbers to the doctor. Then, together with your doctor, review all the medications you are taking: very often they are the cause. At the same time, it is worth ensuring the safety of the house - remove sliding rugs and wires, install handrails in the bathroom, install night lighting, replace slippers with stable shoes with backs: this significantly reduces the risk of a fracture in a fall.
