Bromhidrosis is a persistent unpleasant body odor caused by the decomposition of sweat by skin bacteria. Fresh sweat itself has almost no odor: the smell appears when bacteria process the components of the secretion of the apocrine glands located in the armpits, groin and around the nipples. Therefore, the problem is almost always associated not with a person’s cleanliness, but with a combination of three factors: the amount of sweat, the composition of the skin microflora and the conditions in which it lives - heat, humidity and synthetic fabric. The smell significantly worsens the quality of life and causes anxiety, but in most cases it can be easily corrected.

Where does the smell come from?

There are two types of sweat glands in the skin. Eccrine cells are located throughout the body and secrete almost pure water with salts - this sweat cools the body and has virtually no odor. Apocrines begin to work in adolescence, are found in the armpits, groin, around the nipples and secrete a thicker secretion with proteins and fats. Skin bacteria break it down, and volatile substances with a characteristic pungent odor are formed. The more hair, heat and moisture, the more bacteria multiply and the stronger the smell.

What makes the problem worse

The main amplifier is excessive sweating: the more moisture, the more active the microflora. Excess weight and deep folds of skin, thick synthetic clothing, hot climate and physical work contribute. The smell is affected by nutrition: garlic, onions, hot spices, an abundance of red meat, alcohol and smoking change the composition of sweat. Separately, it is worth mentioning medical reasons: diabetes mellitus, thyroid disease, menopausal hot flashes, some drugs and rare metabolic disorders in which the smell has a specific character.

What you can do yourself

Base - reduce the amount of bacteria and moisture. You should wash daily, or twice in hot weather, paying attention to folds and thoroughly drying the skin. It is helpful to use antibacterial soap or antiseptic products several times a week, trim or remove armpit hair, and wear cotton clothing and change it daily. Antiperspirants with aluminum salts are applied to dry skin at night: this is when they work best, and in the morning you can additionally use deodorant. Clothes with odors are washed at high temperatures.

It is important to understand the other side: people with bromhidrosis often experience severe anxiety, avoid socializing and sports, and are sometimes convinced of the presence of an odor that others cannot smell. Why doesn't deodorant help?+
Deodorant masks odor and partially suppresses bacteria, but does not reduce sweating. For bromhidrosis, you need an antiperspirant with aluminum salts applied to dry skin at night, plus measures against bacteria: antiseptics and hair removal.
Is this due to poor hygiene?+
No. People with bromhidrosis tend to wash themselves even more often than usual. The problem is the composition of the secretion of the apocrine glands and the microflora of the skin, so shower frequency alone cannot solve the problem.
Is it true that smell depends on food?+
Partly yes. Garlic, onions, hot spices, alcohol and smoking change the composition of sweat and increase the smell. The effect varies from person to person, so it makes sense to limit these foods for two weeks and evaluate the difference.
Do botulinum toxin injections help?+
For severe underarm sweating - yes: the drug temporarily blocks the stimulation of the sweat glands, sweat and odor are significantly reduced. The effect lasts on average for several months, after which the procedure is repeated. It is carried out by a doctor.
When is body odor a sign of illness?+
