Furuncle and skin abscess: treatment and autopsy in Tashkent

Other names: Чирей, карбункул, гнойник, абсцесс кожи, фурункулёз

A furuncle is a purulent inflammation of the hair follicle and surrounding tissues, commonly known as a boil. A carbuncle is the fusion of several boils into one large lesion, an abscess is a limited cavity with pus. The causative agent is almost always the same - Staphylococcus aureus. The main rule for all these conditions is easy to formulate: do not squeeze. This is especially true of the face - the area of the nasolabial triangle is connected by veins to the cranial cavity, and an attempt to squeeze out an abscess can result in meningitis or thrombosis of the cerebral sinuses.

With these signs do not wait for a scheduled appointment — the condition requires emergency care.

How it develops

At the infiltration stage, the process can still be terminated conservatively. When the rod has formed, an opening is required - ointments and compresses will not remove pus from the closed cavity.

Why do they appear again?

A single boil is a common situation. Repeated boils (furunculosis) almost always mean there is an underlying cause that needs to be looked for.

  • Diabetes mellitus is checked first; furunculosis is often its first manifestation
  • Carriage of Staphylococcus aureus in the nose is a source of constant self-infection
  • Immunodeficiency conditions, taking immunosuppressants
  • Deficiency of iron, vitamin D, chronic foci of infection
  • Obesity, excessive sweating, friction from clothing
  • Poor hygiene when shaving, skin injury

Treatment

  • At the infiltrate stage: dry heat, antiseptics, if indicated - antibiotic by mouth
  • If an abscess is formed - opening and drainage in a treatment room: under local anesthesia, followed by dressings
  • Antibiotics are required for localization on the face, temperature, multiple lesions, in patients with diabetes and immunodeficiency
  • Culture with sensitivity determination - in case of relapses and ineffectiveness of therapy
  • Sanitation of staphylococcus carriage during furunculosis: treatment of the nasal passages as prescribed by a doctor
  • Compensation for diabetes - without it, relapses are inevitable
Категорически нельзя: выдавливать, прокалывать иглой, распаривать в горячей ванне, накладывать согревающие компрессы на сформировавшийся гнойник, использовать мазь Вишневского на закрытый очаг — всё это способствует распространению гноя в глубину.

Prevention

  • Treat cuts and abrasions with an antiseptic
  • Do not use other people's razors, towels, washcloths
  • Change razor blades, shave according to hair growth, use aftershave products
  • Wear loose clothing made from natural fabrics, change them when sweating
  • Control blood sugar levels
  • For frequent boils - examination by a dermatologist, not self-medication

Frequently asked questions: Furuncle, carbuncle and skin abscess

Is it possible to squeeze out a boil?+
No, under no circumstances. Squeezing pushes the pus deeper and into the bloodstream. On the face, especially in the area of the nasolabial triangle, this can lead to meningitis and thrombosis of the cerebral sinuses.
Why is a boil on the face more dangerous?+
The veins in this area are directly connected to the venous sinuses of the brain, so the infection can spread into the skull. A boil on the face must be treated with antibiotics and under the supervision of a doctor.
Do ointments help?+
At the early stage of infiltration - yes, along with antiseptics. But when an abscess has formed, ointments will not remove pus from a closed cavity: opening and drainage is necessary.
Why do boils appear again?+
Recurrent boils almost always indicate an underlying cause: diabetes mellitus, carriage of staphylococcus in the nose, iron or vitamin D deficiency, immunodeficiency. They need to be found and eliminated.
Can it be heated or steamed?+
Dry heat is only permissible during the initial compaction stage. Steaming and wet warming compresses on a formed abscess are contraindicated - they contribute to the spread of infection.
