Where does blood come from?

Seminal fluid is formed from the secretion of the prostate gland, seminal vesicles and the contents of the vas deferens. Accordingly, the source of blood can be any of these structures, as well as the urethra.

Most often, the seminal vesicles and prostate bleed: these are structures with abundant blood supply and thin vessel walls that are easily damaged by inflammation, increased pressure or after manipulation.

The color of the ejaculate can range from pinkish to dark brown. A dark color usually means the blood has been there for a while and is no more alarming than bright red.

  • Inflammatory: prostatitis, vesiculitis, urethritis, epididymitis - the most common group
  • Sexually transmitted infections
  • Consequences of medical interventions: prostate biopsy, cystoscopy, catheterization, brachytherapy - a very common and completely expected cause

  • Anti-inflammatory treatment for prostatitis and vesiculitis
  • Treatment of sexually transmitted infections with examination of the partner
  • Blood pressure control
  • Correction of anticoagulant therapy together with a cardiologist
  • Removal of prostate stones and seminal vesicle cysts if significant
  • Endoscopic treatment for identified source of bleeding

Is blood in semen cancer?+
In most cases no. In men under forty years of age, the cause is most often inflammation, recent intervention, or an unidentified factor, and the episodes resolve on their own. However, after forty years and with repeated episodes, cancer must be excluded, so examination is mandatory at this age.
What to do in case of a single episode?+
Do not panic and make an appointment with a urologist as planned. The doctor will examine you, prescribe a urine test, examination for infections, and, if you are over forty, PSA and TRUS. In the absence of alarming signs in a young man, observation without any treatment is usually sufficient.
Is this normal after a prostate biopsy?+
Yes, this is an expected phenomenon, not a complication. Blood in semen after a biopsy can persist for weeks or even longer, gradually changing color from red to brown. You will be warned about this before the procedure. You should consult a doctor if there is heavy bleeding, fever, or difficulty urinating.
Is it dangerous for your partner?+
The presence of blood in semen itself does not pose a danger to the partner. However, if the cause is a sexually transmitted infection, then there is a risk of transmitting this particular infection. Therefore, until the cause is determined, it is wise to use a condom, and if an infection is detected, both partners are examined.
Does hemospermia affect conception?+
As a rule, it has no effect on its own. But if it is caused by inflammation of the prostate or seminal vesicles, then this inflammation can worsen the spermogram. Therefore, when planning a pregnancy, it makes sense to take a spermogram and treat the identified inflammation.
Could high blood pressure be to blame?+
Yes, uncontrolled hypertension is considered a possible cause, especially for recurrent episodes in older men. Therefore, the examination includes measuring blood pressure, and its normalization may become part of the treatment.
