VIDEO EEG MONITORING IN THE VITAMED MEDICAL CLINIC

Khusanbaeva Luiza Usmanovna, neurologist-epileptologist, doctor of the highest category at the VITAMED MEDICAL clinic, will talk in this article about Video EEG monitoring.

Many people are horrified when they first see seizures - their symptoms can be so frightening that they can shock an ignorant person.

Even a single attack of seizures requires consultation with a neurologist-epileptologist.

At the first stage, the main task of the epileptologist is to establish an accurate diagnosis.

It should be noted that there are more than twenty different types of epileptic seizures. Each form of epilepsy is characterized by its own “set” of seizures.

The diagnosis of epilepsy is primarily clinical, and is based on a qualified description of seizures.

The similarity of clinical manifestations of different types of seizures and non-epileptic conditions should also be taken into account. All this can lead to underdiagnosis or overdiagnosis of epilepsy. In the first case (underdiagnosis), there are risks of developing severe epileptic seizures and status epilepticus, which can lead to injuries and life-threatening situations (sudden death syndrome, etc.), as well as to impairment of the patient’s cognitive (mental) functions. An erroneous diagnosis of epilepsy (overdiagnosis) can cause serious psychological problems in the patient and lead to social maladaptation, and also the unreasonable prescription of long-term antiepileptic therapy can lead to side effects.

To confirm the epileptic nature of seizures, in most cases, long-term video-EEG monitoring during sleep is required. This method is the only reliable method for diagnosing epilepsy and determining its form.

Video-EEG monitoring (VEM) is a synchronous recording of the bioelectrical activity of the brain (electroencephalography) and a video image of the patient.

An important factor is the duration of the study. A typical EEG study is done in 15-30 minutes. With studies of this duration, as with short video-EEG monitoring, epileptiform activity and epileptic seizures are sometimes missed, which leads to inappropriate drug prescriptions. Such errors can lead to an increase in epileptic seizures or even the development of status epilepticus. Therefore, video EEG monitoring should be quite long.

Properly performed video-EEG monitoring allows:

•          assess the functional state of the brain,

•          record epileptic activity during and between attacks,

•          describe the characteristics of the attack and identify the area of its onset.

Types of video-EEG monitoring:

•          Daytime video-EEG monitoring – duration 1-2-3-4 hours

•          Overnight video-EEG monitoring – duration 8 hours

•          Daily study – duration 24 hours.

Treatment for most forms of epilepsy today is quite effective. The effectiveness of drug treatment depends primarily on the correct selection of drugs. An incorrectly chosen drug will not only not have a positive effect, but may cause an increase in frequency, severity, or the appearance of a new type of attack, delaying the treatment process. Antiepileptic drugs are prescribed only after an accurate diagnosis of epilepsy has been established and the form of the disease has been clarified. Long-term work on selecting drugs and their combinations takes time, but if it is carried out fairly systematically, the results are very good.

At the VITAMED MEDICAL clinic, our patients have the opportunity to undergo video-EEG monitoring examination in comfortable conditions using a complex of the latest highly informative medical equipment.

How is the research itself carried out?

Patients and accompanying persons come to the clinic at the appointed time, where they will be met by specialists who directly conduct the study.  The patient is placed in a comfortable room where special equipment is installed, and a special soft cap with many electrodes is placed on his head.

Typically, the study follows a standard pattern - recording is made in a state of wakefulness before sleep, then sleep, wakefulness after sleep. Functional tests are performed during wakefulness. In children, the sequence may vary slightly depending on the needs of the child. During the study, patients can eat, read, and play. An accompanying person who is in the same roomResearcher, where the patient is, must control the patient’s behavior during the study and ensure that he is constantly in the field of view of the video cameras.

During the study, a long-term synchronous recording of the electroencephalogram, video and audio data on the patient’s motor and behavioral activity is carried out. This study is carried out over several hours, recording the period of sleep.

After completing the study, the patient leaves the room, and the doctor analyzes the recorded data, evaluates the results of their computer processing and, based on all this, issues you a medical opinion.
