Premature ejaculation

On the Internet you can find a huge number of men and women talking about the topic: “why do I cum quickly?”, “...my husband (boyfriend) cums quickly...”, “how to prolong sexual intercourse...” Such complaints are perhaps the most common among problems in the sexual life of men. Thus, the answers to questions about premature ejaculation will be very relevant.

A little theory:

One of the most important differences between humans and animals is their social nature. If in animals sexual intercourse and ejaculation serve mainly for reproduction, then for humans sex is not only a way of procreation, but also part of social relationships.

For a long time, experts tried to determine what premature ejaculation is: they counted frictions, tried to standardize the time of sexual intercourse and, ultimately, came to the general opinion that the duration of sexual intercourse should satisfy the sexual need of both sexual partners.

Thus, premature ejaculation is a condition in which ejaculation occurs constantly or very often earlier than necessary for both sexual partners.

Classification

• Primary PE (existing in the absence of organic diseases of the male genital organs, brain and spinal cord).

• Secondary PE (arising against the background of diseases of the genitourinary organs and central nervous system).

Classification based on the duration of sexual intercourse:

• Absolute PE (duration of sexual intercourse less than 1 minute).

• Relative PE (duration of sexual intercourse is more than 1 minute, but this is not enough for the sexual partner to achieve orgasm).

Causes of premature ejaculation:

• Organic reasons:

o Congenital hypersensitivity of the glans penis.

o Inflammatory diseases of the pelvic organs (prostatitis, colliculitis, vesiculitis).

o Dysfunction of central serotonin receptors.

o Neurological diseases that cause disturbances in the innervation of the pelvic organs (damage to the pelvic bones, spine, intervertebral hernia, damage due to operations on the pelvic organs and spine).

• Psychogenic causes:

o Frequently repeated sexual intercourse or masturbation performed in a hurry during adolescence can lead to the formation of a conditioned reflex leading to accelerated ejaculation

o Lack of self-control experience (beginning of sexual activity)

o Obsessive expectation syndrome (formed as a result of failures in previous sexual activity)

o Early sexual experience.

Diagnostics

Establishing a diagnosis is not difficult: almost all patients describe their condition as: “I’m coming to an end quickly.” It is much more difficult to establish the form of the disease. Diagnosis is based on a detailed interview with the patient, identifying inflammatory diseases of the prostate gland, seminal vesicles, spinal diseases, and organic brain lesions. Based on the data obtained, the form of premature ejaculation is determined.

Distinctive features of various forms of PE:

• Increased sensitivity of the glans penis is characterized by a relatively constant duration of sexual intercourse; the duration of sexual intercourse rarely increases with the appearance of regular sexual activity; the second, third and subsequent sexual intercourse only slightly prolong the time of coitus; alcohol and lubricants with anesthetic significantly prolong sexual intercourse; ejaculation never occurs before the penis is inserted into the vagina.

• Premature ejaculation against the background of inflammatory diseases (prostatitis, colliculitis, vesiculitis) is characterized by a wave-like course; appears suddenly, against the background of a satisfactory duration of sexual intercourse, even in adolescence; ejaculation may occur before sexual intercourse; subsequent sexual intercourse is usually longer; alcohol and lubricants with anesthetics do not affect the duration of sexual intercourse, or have a slight effect; sometimes this form is accompanied by blurred orgasmic sensations and pain.

• Psychogenic premature ejaculation is different in that it is more typical for boys and men who do not have regular sex life; the duration of sexual intercourse can vary widely; ejaculation can be significantly delayed due to distracted or unpleasant thoughts during sexual intercourse; Even a small amount of alcohol can significantly prolong the time of sexual intercourse.

Treatment of premature ejaculation - how to prolong sexual intercourse?

Treatment methods are determined by the reasons for short sexual intercourse. Can be used:

• Pharmacological prolongation of sexual intercourse (menatural anesthetics, alpha-blockers, serotonin reuptake inhibitors, herbal medicine).

• Formation of a conditioned reflex, the opposite of the one formed in adolescence (start-stop technique).

• Treatment of inflammatory diseases of the pelvic organs (prostatitis, vesiculitis, colliculitis).

• Physiotherapeutic methods for prolonging sexual intercourse (electrical stimulation of the penis).

• Surgical treatment (circumcision, denervation-reinnervation of the glans penis, penile prosthesis).

It is important to understand that treating accelerated ejaculation is sometimes a lengthy process and may require patience from the man and his sexual partner.
