Experience in using surgical laser in otorhinolaryngology

Otorhinolaryngologists of the clinic VITAMED MEDICAL Isakov Ulugbek Khasanovich and Khudayberganov Said Igamberdievich share their experience in using surgical laser in otorhinolaryngology.

The use of laser radiation in medicine began in 1964. Today, the field of application of lasers in medicine is very wide and includes almost all areas.

Laser surgery in otorhinolaryngology originated in the seventies of the twentieth century. In the beginning, the carbon dioxide laser historically dominated. Its significance continues to this day. In recent years, types of lasers whose radiation can be transmitted through optical fibers (Nd:YAG, diode lasers) have received special development. The contact operating technique gave impetus to the emergence of new opportunities in endoscopic ENT surgery. In Uzbekistan, the first experiences of use began in 2012 in some private clinics, and since 2014, public medical centers began to widely use laser surgery techniques. On the basis of the Institute for Advanced Training of Doctors, foreign internship courses on the use of laser technology in surgery were organized.

Laser technology makes it possible to avoid bleeding and the formation of postoperative swelling of soft tissues, since, by cutting tissue, the laser simultaneously “seals” blood vessels and provides a pronounced antiseptic effect, which is very important for the healing process, in contrast to monopolar and bipolar coagulation.

Minimally invasiveness, low risk of bleeding, sterilizing effect of the laser, high precision during contact work with a flexible fiber light guide and minimal postoperative reactive phenomena have become the basis for the widespread use of diode surgical laser in the treatment of diseases of the ENT organs - hypertrophic and vasomotor rhinitis, polyps, scars and synechiae of the nasal cavity, recurrent nosebleeds, ronchopathy, chronic tonsillitis, chronic pharyngitis, cysts and papillomas of the pharyngeal cavity.

Laser surgery has found its wide application in the VITAMED MEDICAL clinic. Our surgical doctors successfully use laser surgery. Laser treatment of pathologically altered tissues is an effective method for treating chronic diseases of the ENT organs.

Advantages of laser surgery compared to traditional methods:

· The operation is performed under local or general anesthesia

Laser makes it possible to perform surgery with minimal blood loss

· 2 hours after the operation the patient goes home

· Short recovery rehabilitation period after surgery - re-examination is carried out after 2-3 days.

Which ENT diseases can be completely cured by laser surgery:

Vasomotor rhinitis

Hypertrophic rhinitis

Synechiae of the nasal cavity

Benign formations of the oropharyngeal mucosa - cysts, papillomas

Hypertrophy (proliferation) of lymphatic granules of the posterior pharyngeal wall, lateral pharyngeal ridges

Constant (recurrent) nosebleeds

Chronic tonsillitis

In our surgical practice we use the Biolitec diode laser. The diode laser allows for both superficial and interstitial destruction of the cavernous tissue of the inferior turbinates. Interstitial exposure does not damage the ciliated epithelium. The operation can be performed under general or local infiltration anesthesia, radiation power 5-8 W. Depending on the degree of hypertrophy, we make 2-4 interstitial channels. Reactive edema is minor, lasting up to 3 days.

At the VITAMED MEDICAL clinic from 2016 to 2017. About 75 such operations were carried out. There were no intra- or postoperative complications, and no re-interventions were required. Reactive postoperative phenomena persist for a maximum of 3-4 days.

The use of laser surgical methods alone in nasal surgery is not always possible due to a number of factors, for example, with endoscopic polypotomy. Multiple polyps with almost complete obstruction of the nasal passages, difficulties in visualizing the stalk of the polyp due to the pathology of the intranasal structures, etc. Therefore, we prefer to remove multiple nasal polyps with a microdebrider (shaver) under endoscopic control, followed by laser coagulation of the remnants of polyposis-altered tissue. The clinic performed about 64 such operations between 2016 and 2017. In the postoperative period, a six-month course of topical steroid therapy was prescribed. The frequency of relapses depends on the degree of spread of polyps accompanying pathologyology (bronchial asthma) and the accuracy of patients’ compliance with medical prescriptions.

Thus, 2 years of experience in our clinic once again proves the advantages of using laser surgical techniques, both as primary and auxiliary, in outpatient otorhinolaryngology, confirming their reliability and safety.
