Surgery is the only effective treatment for varicocele. Its main goal is the intersection of all dilated venous vessels going into the pampiniform plexus, through which the reverse flow of blood occurs. The most preferred method of treating varicocele is microsurgical ligation from an infrainguinal mini-access. The operation is performed using special microsurgical instruments. After dissecting the skin and subcutaneous fatty tissue, the surgeon finds the spermatic cord and ties its veins under the control of a microscope. Optical magnification of all structures of the spermatic cord allows the vein ligation procedure to be carried out as correctly as possible, without injuring the nerves, lymphatic vessels, or arteries. Moreover, it is possible to ligate the inguinal venous collaterals, as well as the cremasteric vein and veins of the testicular guide ligament along with the internal spermatic vein, which runs as part of the spermatic cord.

Hospitalization in a hospital is not required; the patient can leave the clinic and return home on the same day. The surgical intervention itself is performed relatively quickly; all manipulations require about forty minutes. After this, the patient remains under the supervision of a doctor for several hours. Rehabilitation usually takes two days, in rare situations more.

In fact, there is currently no other, more modern method of treating varicocele than microsurgical varicocelectomy. It allows a man not to change his usual lifestyle after surgery.

According to world practice, microsurgical varicocelectomy almost twice as often leads to the restoration of impaired reproductive function of a man. According to information released by Mark Goldstein, a professor from New York Hospital, who analyzed the results of eliminating varicoceles using microsurgery in one and a half thousand patients, pregnancy in the observed married couples occurred in 43% of cases within one year and in 69% of cases two years after this operation. Of this total number, only fourteen people (less than 1%) had recurrences of varicocele. Neither testicular hydrocele (hydrocele) nor testicular atrophy was noted.
